摘　　要：目的对2 cm以下磨玻璃结节(GGN)的空气支气管征进行分析,提高对GGN的诊断与鉴别诊断能力。方法回顾性分析经手术病理证实的2 cm以下GGN共63例,均为孤立性结节。16例为纯磨玻璃结节(pGGN),47例为混合磨玻璃结节(mGGN)。分析不同病理结果与空气支气管征的关系,将空气支气管征分为3型:Ⅰ型,支气管在病灶内走行正常(分叉或不分叉);Ⅱ型,支气管在病灶内扭曲、扩张;Ⅲ型,病灶内支气管管壁局部增厚或截断。结果pGGN组Ⅰ型、Ⅱ型、Ⅲ型空气支气管征显示率分别为3/16(18.8%)、1/16(6.3%)、0/16(0%);mGGN组Ⅰ型、Ⅱ型、Ⅲ型空气支气管征显示率分别为18/47(38.3%)、6/47(12.8%)、9/47(19.1%)。癌前病变与浸润性病变的空气支气管征显示率分别为2/7(28.3%)、35/56(62.5%)。结论对于2 cm以下GGN,空气支气管征分型在pGGN组及mGGN组有差别,有助于对其浸润程度进行分析。Objective To analyze the air bronchogram of malignant ground-glass nodules(GGN)with a size of≤2 cm,to improve the diagnosis and differential diagnosis ability of GGN.Methods 63 solitary GGN(pGGN 16 cases,mGGN 47 cases)confirmed pathologically were analyzed retrospectively.The relationship between different pathological results and air bronchogram was analyzed.The air bronchogram was classified into three types:TypeⅠ,the bronchus was normal in the nodule;TypeⅡ,the bronchus was dilated and distorted in the nodule;TypeⅢ,the bronchus was thickened or interrupted in the nodule.Results For pGGN,the rates of typeⅠ,typeⅡand typeⅢair bronchogram were 3/16(18.8%),1/16(6.3%)and 0/16(0%)respectively;For mGGN,the rates of typeⅠ,typeⅡand typeⅢair bronchogram were 18/47(38.3%),6/47(12.8%)and 9/47(19.1%)respectively.The rates of precancerous lesions and invasive adenocarcinoma were 2/7(28.3%),35/56(62.5%),respectively.Conclusion For the GGN of 2 cm or less,the type of air bronchogram between pGGN and mGGN is different,which may be helpful for prediction the degree of invasion.
