摘  要:目的:评估EGFR突变的具体类型是否与非小细胞肺癌(NSCLC)患者手术后复发或转移的具体部位有关。方法:回顾性收集2009年至2012年间在我院胸外科接受手术治疗后复发或者转移的NSCLC患者,共187例。评估NSCLC患者EGFR突变与否是否与患者的无瘤生存率相关,EGFR基因突变的类型是否与肿瘤复发和转移的具体部位有关。结果:187例NSCLC患者中,58例患者产生了EGFR基因的突变,突变比例为31.0%;EGFR突变的肺癌中,腺癌占86.2%。EGFR是否突变对于患者的无瘤生存时间无明显影响,但不同的EGFR突变类型与无瘤生存时间之间有显著差异,19外显子突变患者容易发生骨转移,T790M突变患者容易出现脑转移,而L858R患者发生肝转移比例较大。结论:EGFR不同突变类型的NSCLC患者在手术后有不同的预后,与特定部位的复发转移有关。Objective:To evaluate if specific EGFR mutations could predict site-specific recurrence and metastasis following non-small-cell lung cancer(NSCLC)surgery.Methods:187 NSCLC patients,who underwent pulmonary lobectomy and recurrence/metastasis,were retrospectively collected.The association of EGFR mutation with tumor-free survival and site-specific recurrence and metastasis was calculated.Results:Among 187 cases,EGFR genes in 58 patients were mutated,covering 31.0%.Adenocarcinoma was the main type of EGFR-mutated NSCLC,covering 86.2%.EGFR mutation had no relationship with tumor-free survival,but tumor-free survival among different mutations were significantly different.Patients with exon mutations were prone to bone metastasis,and patients with T790M mutations were prone to brain metastases,while L858R patients had a greater proportion of liver metastases.Conclusion:NSCLC patients with different EGFR mutations have different tumor-free survival,associated with different site of recurrence.
