奥曲肽联合乌司他丁治疗重症胰腺炎患者的疗效及其对免疫功能的影响

摘　　要：目的探讨奥曲肽联合乌司他丁治疗重症胰腺炎患者的临床效果及其对免疫功能的影响。方法选取2015年2月至2018年3月于潮州市中心医院治疗的重症胰腺炎患者68例,按随机数字表法分为单药组和联合组,各34例。单药组行奥曲肽治疗,联合组在单药组基础上加用乌司他丁治疗。比较两组患者临床疗效、免疫指标(IgA、IgG及IgM)变化情况及不良反应发生情况。结果联合组治疗有效率明显高于单药组,不良反应发生率明显低于单药组(P<0.05);治疗后,两组患者IgA及IgM水平比较,差异无统计学意义(P>0.05),但联合组IgG水平明显高于单药组(P<0.05)。结论奥曲肽联合乌司他丁治疗重症胰腺炎患者可有效改善患者IgG水平,且临床疗效显著。Objective To investigate the clinical effects of octreotide combined with ulinastatin in the treatment of patients with severe pancreatitis and its immune function.Methods A total of 68 patients with severe pancreatitis treated in Chaozhou Central Hospital of Guangdong Province from February 2015 to March 2018 were selected according to the random number table method,and were divided into a single drug group and a combined group,with 34 cases in each group.The single drug group was treated with octreotide,and the combined group was treated with ulinastatin on the basis of the single drug group.The clinical efficacy of the 2 groups of patients,the changes of immune indexes(IgA,IgG and IgM)and the occurrence of adverse reactions were compared.Results The clinical efficacy of the combined group was significantly higher than that of the single drug group,and the incidence of adverse reactions was significantly lower than that of the single drug group(P<0.05);after treatment,there was no significant difference in the levels of IgA and IgM between the 2 groups(P>0.05),but the IgG level in the combined group was significantly higher than that in the single drug group(P<0.05).Conclusion Octreotide combined with ulinastatin in the treatment of patients with severe pancreatitis can effectively improve IgG levels with significant efficacy.
