激光周边虹膜成形术治疗原发性急性闭角型青光眼的有效性及对眼压的影响

摘　　要：目的探讨激光周边虹膜成形术治疗原发性急性闭角型青光眼的有效性及对眼压的影响。方法将96例(96眼)原发性急性闭角型青光眼患者按治疗方案不同分为对照组和研究组,各48例(48眼)。对照组给予传统的紧急药物治疗,研究组行激光周边虹膜成形术治疗。比较两组的治疗效果。结果治疗后,两组患者的虹膜厚度均较治疗前减小,虹膜夹角、房角开放距离、中央前房深度均较治疗前增大,且研究组优于对照组(P<0.05)。研究组治疗后2、6 h的眼压均低于对照组及治疗前(P<0.05)。研究组治疗后的视力优于对照组,甘露醇用量、降眼压所用时间均少于对照组(P<0.05)。结论采用激光周边虹膜成形术治疗原发性急性闭角型青光眼,可迅速降低患者的眼压,有效开放房角,促进视力恢复。Objective To investigate the effectiveness of laser peripheral iridoplasty on primary acute angle closure glaucoma and its influence on intraocular pressure.Methods Ninety-six patients(96 eyes)with primary acute angle closure glaucoma were divided into control group and study group according to different treatment schemes,with 48 cases(48 eyes)in each group.The control group was given traditional emergency drug treatment,and the study group was treated with laser peripheral iridoplasty.The therapeutic effects of the two groups were compared.Results After treatment,the iris thickness in the two groups decreased,the iris angle,the angle opening distance and the depth of central anterior chamber increased,and those of the study group were better than the control group(P<0.05).The intraocular pressure at 2 and 6 hours after treatment of the study group were lower than those of the control group and before treatment(P<0.05).After treatment,the visual acuity of the study group was better than that of the control group,and the dosage of mannitol and the time of intraocular pressure reduction were less than those of the control group(P<0.05).Conclusion Laser peripheral iridoplasty on primary acute angle closure glaucoma can rapidly reduce the intraocular pressure,effectively open the angle of the chamber,and promote the recovery of vision.
