摘　　要：目的探讨卵巢浆黏液性肿瘤的临床、影像、病理学特征及诊断意义。方法回顾性分析23例卵巢原发浆黏液性肿瘤的临床、影像及病理资料,总结其影像学及组织学形态特点。结果23例浆黏液性肿瘤患者年龄23~70岁,中位年龄44岁。其中发生在左侧9例,发生在右侧12例,双侧发生2例。共25个肿块在CT机断层扫描或磁共振成像中发现,病理结果证实16个良性,8个交界,1个恶性。12个为单囊病灶,信号均匀,6个可见囊内壁结节,1个为实性。良性肿瘤表现多为单房或多房囊性肿块,为囊壁光整,囊内分隔薄而规则。交界性或恶性表现为囊壁及囊内分隔厚而不规则,可见囊内壁结节。交界性肿瘤中实性部分的T2WI呈高信号,囊内液体T1WI呈高信号,T2WI上呈低信号。结论浆黏液肿瘤的影像学特征因人而异,CT和MRI在良性、交界性及恶性浆黏液性肿瘤的诊断和鉴别诊断中具有重要的价值。Objective To investigate the clinical,imaging,pathological features and diagnostic significance of ovarian seromucinous tumors.Methods The clinical,imaging and pathological data of 23 cases of primary seromucinous tumor of ovary were analyzed retrospectively.Results The age of 23 patients with seromucinous tumors was 23-70 years(median age 44).Among them,8 cases only occurred on the left side,13 cases only occurred on the right side,and 2 cases occurred bilaterally.A total of 25 masses were found in CT or MRI.The pathological results showed that there were 16 benign masses,8 borderline and 1 cancer.Twelve were single cystic lesions with uniform signal,6 were multilocular cystic lesions with septum in the capsule,which were multi-locular and cystic,and the signal of the lesions were mostly uneven.There were six intrathecal nodules and 1 solid.Benign tumors are usually single chamber or multi chamber cystic masses,with smooth wall and thin and regular separation.The borderline or malignant manifestations were thick and irregular septum of the wall and the inside of the capsule.In the solid part of the borderline tumors,T2WI showed high signal,fluid T1WI showed high signal,and T2WI showed low signal.Conclusion The imaging characteristics of seromucinous tumors vary from person to person.CT and MRI have important value in the diagnosis and differential diagnosis of benign,borderline and malignant seromucinous tumors.
