D-二聚体 纤维蛋白原 超敏C反应蛋白与脑梗死后非痴呆性血管性认知障碍的关系

摘　　要：目的探讨血D-二聚体、纤维蛋白原、超敏C反应蛋白与脑梗死后非痴呆性血管性认知障碍的关系。方法测定50例非痴呆性血管性认知障碍患者及50例健康对照组的D-二聚体、纤维蛋白原、超敏C反应蛋白水平。采用蒙特利尔认知评估(MoCA)量表对50例非痴呆性血管性认知障碍患者及50例健康对照组进行评估。结果非痴呆性血管性认知障碍患者D-二聚体水平是(0.85±0.26)mg/L,纤维蛋白原水平是(3.10±0.85)g/L,超敏C反应蛋白水平是(11.06±4.15)mg/L。健康对照组的D-二聚体水平是(0.30±0.12)mg/L,纤维蛋白原水平是(2.30±0.31)g/L,超敏C反应蛋白水平是(3.83±0.76)mg/L。非痴呆性血管性认知障碍患者的D-二聚体、纤维蛋白原、超敏C反应蛋白均高于健康对照组,差异有统计学意义(P<0.05)。相关性分析显示D-二聚体、纤维蛋白原、超敏C反应蛋白与MoCA评分呈正相关(P<0.05)。结论血D-二聚体、纤维蛋白原、超敏C反应蛋白的升高可能是非痴呆性血管性认知障碍的危险因素,降低血D-二聚体、纤维蛋白原、超敏C反应蛋白水平可能可以延缓非痴呆性血管性认知障碍进展为血管性痴呆。Objective To study the relationship between D-dimer(DD),fibrinogen(FIB),high sensitivity C-reactive protein(hs-CRP)and vascular cognitive impairment no dementia(VCIND)after cerebral infarction.Methods Serum DD,FIB,hs-CRP was measured in 50 VCIND patients and 50 normal controls.The neuropsychological scores were obtained by Montreal Cognitive Assessment(MoCA)scales in 50 VCIND patients and 50 normal controls.Results Serum DD,FIB,hs-CRP levels of VCIND patients was(0.85±0.26)mg/L,(3.10±0.85)g/L,(11.06±4.15)mg/L.Serum DD,FIB,hs-CRP levels of normal controls was(0.30±0.12)mg/L,(2.30±0.31)g/L,(3.83±0.76)mg/L.The levels of serum DD,FIB,hs-CRP was significantly higher than that in normal controls(P<0.05).The levels of DD,FIB,hs-CRP in VCIND patients were positive correlated with MoCA scores(P<0.05).Conclusion Elevation of serum DD,FIB,hs-CRP may be risk factor for VCIND.The progression rate from VCIND to vascular dementia could be delayed by reduction of DD,FIB,hs-CRP.
