摘　　要：目的观察A型肉毒素联合托吡酯治疗慢性偏头痛的有效性及安全性。方法选取慢性偏头痛患者56例,按随机数字表分为BTX-A组、TPM组、联合治疗组。随访记录各时间段(治疗前、治疗后1、2、3、6个月)各观察指标,同时记录相关不良反应,评价各组用药的安全性。结果各组治疗后各观察时间点每月偏头痛发作频率、头痛发作持续时间、疼痛视觉模拟评分、服用止痛药数量较治疗前明显改善。联合治疗组每月头痛发作频率、头痛持续时间、疼痛视觉模拟评分、服用止痛药数量与BTX-A组及TPM组相比差异有统计学意义(P<0.05)。各组不良反应比较差异无统计学意义(P>0.05)。结论A型肉毒素联合托吡酯治疗慢性偏头痛安全有效。Objective To observe the effectiveness and safety of botulinum toxin type A combined with topiramate in the treatment of chronic migraine.Methods A total of fifty-six patients with chronic migraine were divided into three groups according to the random number table,botulinum toxin type A,topiramate group and combined treatment group.All data were recorded at all the different times(before treatment,after treatment for 1 month,2 months,3 months and 6 months).All data was statistically analyzed.At the same time,the drug-related side effects were recorded to evaluate the safety of medication in each group.Results Frequency of migraine attack,headache duration,score of visual analogue scale,number of painkillers were significantly improved at different times after treatment compared with those before treatment.There was a statistically significant difference in the frequency of headache attacks,the duration of headache,score of visual analogue scale,and the number of painkillers in the combined treatment group compared with the botulinum toxin and topiramate treatment groups.There were no significant differences in drug-related side effects among the three groups.Conclusion Botulinum toxin type A combined with topiramate was safe and effective in the treatment of chronic migraine.
