摘　　要：目的探讨使用经气道咳痰机对于神经肌肉病变机械通气患者的呼吸力学参数以及脱机成功率的影响。方法在支持治疗的基础之上,对2018年2月至2019年12月入重症医学科的60例神经肌肉病变行有创机械通气住院患者随机分为治疗组30例、对照组30例,治疗组采取咳痰机连接气管插管或气管切开模拟生理性咳嗽方法,对照组采取手动叩背方法。比较两组患者应用机械通气的时间的差异,经过不同排痰方式吸痰后肺的顺应性、气道阻力的差异。结果对照组机械通气时间、患者吸痰后的气道阻力大于治疗组、肺的顺应性小于治疗组。结论经气道使用咳痰机咳痰方法应用于神经肌肉病变患者的气道廓清,能够降低患者的气道阻力以及改善肺部的顺应性,降低机械通气的时间和住ICU时间。Objective To investigate the effect of airway expectoration machine on respiratory mechanics parameters and success rate of weaning in the patients with mechanical ventilation caused by neuromuscular lesions.Methods On the basis of supportive treatment,60 inpatients with invasive mechanical ventilation because of neuromuscular lesions treated in Department of Critical Care Medicine from February 2018 to December 2019 were randomly divided into observation group(30 cases)and control group(30 cases).In the treatment group,expectoration machine was used to connect tracheal intubation or tracheotomy was performed to simulate physiological cough.The treatment group was treated with manual percussion on the back.The differences in the time of mechanical ventilation,airway resistance and lung compliance after sputum aspiration in different ways of sputum excretion were compared between the two groups.Results The control group owned longer mechanical ventilation time and more airway resistance after sputum aspiration than the treatment group,and the lung compliance of the former group was less than that of the latter group.Conclusion The airway clearance for patients with neuromuscular lesions by airway expectoration machine can reduce airway resistance,improve lung compliance,and shorten the time of mechanical ventilation and ICU stay.
