摘 要:目的探讨miR-19b在上皮性卵巢癌患者血小板中的表达及其潜在临床意义。方法收集2018年6月至2019年10月广东省第二人民医院住院治疗的上皮性卵巢癌(epithelial ovarian cancer,EOC)患者40例,依据国际妇产科联盟(FIGO)卵巢癌手术病理分期分为FIGOⅠ-Ⅱ期组、FIGOⅢ-Ⅳ期组。血栓弹力图检测血小板聚集功能,流式细胞术检测血小板CD62P(P-selectin)的表达,实时荧光定量PCR法检测血小板miR-19b的表达。结果与FIGOⅠ-Ⅱ期组相比,FIGOⅢ-Ⅳ期的血小板计数、MA值、CI值、血小板CD62P阳性率和血小板miR-19b表达水平等明显升高(P≤0.001),R值、K值降低(P <0.05),Angle值无明显变化(P=0.332)。相关性分析显示:血小板miR-19b表达水平分别与MA、CI、血小板计数、血小板CD62P阳性率、CA125、DVT事件呈正相关(P≤0.001),与K值呈弱负相关(r=-0.336,P=0.034),与R、Angle值无明显相关(P> 0.05)。结论晚期上皮性卵巢癌患者血小板数量增多、聚集和活性功能增强,血小板miR-19b表达上调,提示miR-19b可能参与调控了血小板的促EOC转移作用。Objective This research aimed to explore the miR-19b expression of platelets in patients with epithelial ovarian cancer and its relationship with platelet activation and coagulation function. Methods 40 cases of epithelial ovarian cancer(EOC)were collected from June 2018 to October 2019 in Guangdong Second Provincial General Hospital,and were divided into federation international of gynecology and obstetrics(FIGO)Ⅰ-Ⅱphase group,FIGO Ⅲ-Ⅳ phase group. Thrombelastogram was applied to detect platelet aggregation function,Flow-cytography was adopted to detect the expression of platelet CD61 and CD62P(P-selectin),real-time fluorescence quantitative PCR was used to detect the platelet miR-19b expression. Results Compared with the FIGO Ⅰ-Ⅱ phase group,platelet count,MA value,CI value,platelet CD62P positive rate and miR-19b expression were significant increased(P < 0.001),R value and K value were significantly reduced(P < 0.05)and Angle value was no significant changed(P = 0.332)in FIGO Ⅲ-Ⅳ phase group. Correlation analysis showed that platelet miR-19b was significantly positively correlated with MA,CI,platelet count and platelet CD62P positive rate,CA125,and DVT events(P ≤0.001). It was weak negatively correlated with K values(r=-0.336,P=0.034)and not correlated with R and Angel values(P > 0.05). Conclusion The platelet count,activity,aggregation and miR-19b expression were up-regulated in patients with advanced epithelial ovarian cancer,suggesting that miR-19b may play an important role in platelet involvement in promoting the progression of EOC.
