摘 要:目的分析牙龈卟啉单胞菌(Pg)在食管癌前病变进展为癌过程中的作用。方法选取2007年2月至2012年12月经病理诊断为食管癌前病变的患者73例,随访5 a后进展为食管鳞癌者33例为观察组,仍为癌前病变者40例为对照组。免疫组化SP法检测在5 a前后组织中两组Pg的表达情况并进行分析。结果观察组5 a前Pg阳性率为42.4%,免疫组化评分为6.33,5 a后Pg阳性率为54.5%,免疫组化评分为10.17,组内免疫组化评分比较差异有统计学意义(P<0.05);对照组在5 a前Pg阳性率40.0%,免疫组化评分2.93,5 a后Pg阳性率45.0%,免疫组化评分7.77,组内免疫组化评分比较差异有统计学意义(P<0.05);5 a前观察组Pg阳性率和免疫组化评分均明显高于对照组(P<0.05);5 a后观察组Pg阳性率和免疫组化评分也明显高于对照组(P<0.05)。结论Pg感染促进了食管癌前病变进展为食管癌的过程,Pg可能是食管癌变过程的一个重要危险因子。Objective To investigate whether Porphyromonas gingival(Pg)plays a key role in promoting the development of precancerous esophageal lesions into cancer.Methods 73 patients with precancerous lesions from 2007 year to 2012 year were included retrospectively,who were divided into the observatory group(33 cases,who were with precancerous lesions before 5 years and developed into cancer after 5 years)and the control group(40 cases,who were with precancerous lesions only during 5 years).Immunohistochemical method was used to detect the expression of Pg in the 2 groups before and after 5 years.Results In the observatory group,the positive rate of Pg infection was 42.4%and the immunohistochemical score was 6.33 before 5 years;the positive rate of Pg infection was 54.5%and the immunohistochemical score was 10.17 after 5 years.There was a significant difference in the group(P<0.05).In the control group,the positive rate of Pg infection was 40.0%and the immunohistochemical score was 2.93 before 5 years;the positive rate of Pg infection was 45.0%and the immunohistochemical score was 7.77 after 5 years.There was a significant difference in the group(P<0.05).The Pg positive rate and the immunohistochemical score in the observatory group were significantly higher than those in the control group before or after 5 years(P<0.05).Conclusion Pg infection promotes the progression of esophageal precancerous lesions to esophageal cancer.Pg may be an important risk factor in the esophageal carcinogenesis.
