胃癌患者Hp感染与CyclinD1、NF-kb、Beclin1表达及临床病理特征的关系分析

摘　　要：目的探讨胃癌患者幽门螺旋杆菌(Hp)感染与细胞周期蛋白D1(CyclinD1)、核因子kb(NF-kb)、Beclin1表达及临床病理特征的关系。方法选取胃癌患者80例,检测是否Hp感染,分析Hp感染与CyclinD1、NF-kb、Beclin1表达、临床病理特征的关系。结果Hp感染率为68.75%。Hp感染患者CyclinD1、NF-kb、Beclin1阳性率分别为76.36%、63.64%、60.00%,明显高于非Hp感染患者的48.00%、12.00%、36.00%(P<0.05)。胃癌患者Hp感染与CyclinD1阳性率、NF-kb阳性率、Beclin1阳性率呈正相关(P<0.05)。临床分期Ⅰ-Ⅱ期的Hp感染率为50.00%,明显低于Ⅲ-Ⅳ期的84.09%(P<0.05),浸润程度T1+T2的Hp感染率为58.49%,明显低于T3+T4的88.89%(P<0.05)。经二元Logistic回归分析,临床分期、浸润程度为胃癌患者Hp感染的独立影响因素(P<0.05)。结论胃癌患者Hp感染可能通过诱发CyclinD1、NF-kb、Beclin1异常参与胃癌的发生发展,Hp感染与临床分期和浸润程度有关,对胃癌的预防和治疗具有重要意义。Objective To investigate the relationship between Helicobacter pylori(Hp)infection and the expression of cyclin D1(CyclinD1),nuclear factor kb(NF-kb),Beclin1and clinicopathological characteristics in gastric cancer patients.Methods 80patients with gastric cancer were selected to detect Hp infection,and the relationship between Hp infection and CyclinD1,NF-kb,Beclin1expression,and clinical pathological characteristics was analyzed.Results Hp infection rate was 68.75%.The positive rates of CyclinD1,NF-kb,and Beclin1in patients with Hp infection were 76.36%,63.64%,and 60.00%,which were significantly higher than those in patients without Hp infection(48.00%,12.00%,and 36.00%)(P<0.05).Hp infection in gastric cancer patients was positively correlated with the positive rate of CyclinD1,the positive rate of NF-kb,and the positive rate of Beclin1(P<0.05).The Hp infection rate in clinical stages Ⅰ-Ⅱ was 50.00%,which was significantly lower than 84.09%in stage Ⅲ-Ⅳ(P<0.05).The Hp infection rate of infiltration degree T1+T2was 58.49%,which was significantly lower than that of T3+T4.88.89%(P<0.05).By binary logistic regression analysis,clinical stage and degree of invasion were independent factors of Hp infection in patients with gastric cancer(P<0.05).Conclusion Hp infection in gastric cancer patients may be involved in the occurrence and development of gastric cancer by inducing CyclinD1,NF-kb,Beclin1abnormalities.Hp infection is related to clinical stage and degree of invasion,and has important significance for the prevention and treatment of gastric cancer.
