老年周围型肺癌超声造影模式与细胞分化程度的相关性研究

摘　　要：目的探讨老年周围型肺癌超声造影模式与细胞分化程度的相关性研究。方法选取2018年1月—2019年8月在我院经病理检查确诊为周围型肺癌的老年患者84例,所有患者均进行彩色多普勒超声检查,并将其超声造影模式与细胞分化程度进行对照研究。结果不同病理类型下肺癌结节与超声造影增强程度及开始增强形态比较,(P<0.05),与增强分布特征、首先增强部位、填充方式比较,(P>0.05)。不同分化程度肺腺癌、肺鳞癌与肺动脉早期、肺动脉晚期超声造影增强程度比较,(P<0.05),与支气管动脉期增强程度、增强分布特征、首先增强部位、填充方式等超声造影表现比较,(P>0.05)。结论周围型肺癌超声造影模式与细胞分化程度存在相关性,肺动脉早期、等增强的肺癌组织倾向于高分化程度,肺动脉晚期、弱增强的肺癌组织倾向于低分化程度,具有较高的术前诊断价值,值得临床进一步研究。Objective To investigate the correlation between contrast-enhanced ultrasonography(CEUS)mode and cell differentiation in elderly peripheral lung cancer.Methods The 84elderly patients with peripheral lung cancer diagnosed by pathological examination in our hospital from January 2016to January 2019were selected.All patients were examined by color Doppler ultrasonography,and their contrast-enhanced ultrasonography patterns were compared with the degree of cell differentiation.Results The enhancement degree and initial enhancement morphology of lung cancer nodules under different pathological types were compared with those of contrast-enhanced ultrasound(P<0.05),and with the characteristics of enhancement distribution,first enhancement site and filling mode(P>0.05).Contrast-enhanced ultrasonography(CEUS)was compared between adenocarcinoma of lung and squamous cell carcinoma of lung with different degree of differentiation,early and late pulmonary artery(P<0.05),contrast-enhanced ultrasonography was compared with bronchial artery enhancement,enhancement distribution,first enhancement site and filling mode(P>0.05).Conclusion Contrast-enhanced ultrasonography of peripheral lung cancer is correlated with the degree of cell differentiation.The lung cancer tissues with early and equal enhancement of pulmonary artery tend to be highly differentiated,while those with late and weak enhancement of pulmonary artery tend to be poorly differentiated.It has high preoperative diagnostic value and is worthy of further clinical study.
