大黄牡丹汤保留灌肠对小儿急性阑尾炎腹腔镜术后围手术期加速康复疗效观察

摘　　要：目的:观察大黄牡丹汤灌肠对小儿急性阑尾炎腹腔镜术后围手术期加速康复的观察。方法:选择急性阑尾炎患儿258例,随机分为治疗组和对照组,对照组为腹腔镜常规手术治疗,治疗组在腹腔镜手术治疗后给予大黄牡丹汤保留灌肠。比较两组疗效,观察术后胃肠功能恢复及腹腔炎症控制的情况。结果:治疗组总有效率为96.43%,对照组总有效率为89.73%,治疗组总有效率高于对照组,组间比较差异有统计学意义(P<0.05)。治疗组术后排气时间、排便时间、腹腔引流管拔除时间、住院天数、超敏C反应蛋白均较对照组少,组间比较差异有统计学意义(P<0.05)。结论:大黄牡丹汤保留灌肠对小儿急性阑尾炎腹腔镜术后围手术期疗效确切,能有效加速恢复胃肠功能,减轻术后腹腔炎性反应,缩短住院时间。Objective:To observe the effect on enhanced recovery in Dahuang Mudan decoction retention enema for acute appendicitis in Children with in perioperative period after laparoscopic operation.Methods:258 cases hospitalized children with acute appendicitis were randomized into treatment group and control group.The control group was given laparoscopic surgery treatment,while the treatment group was treated with Dahuang Mudan decoction retention enema.The therapeutical effects of both groups were comparable according to recovering time of gastrointestinal function and controlled time of abdominal inflammation.Results:The total effective rates of treatment group and control group were 96.43%and 89.73%with statistically significant difference(P<0.05).The treatment group,the recovery time of bowel sound,defecaion time,time of remove the abdominal drainage tube,total hospitalization time was shorter than the controlled group,and the rate of hs-CRP was lower than the controlled group,the differences were statistically significant(P<0.05).Conclusion:It has curative effect of using Dahuang Mudan decoction combined with laparoscopy in treating acute appendicitis,appendicitis,can significantly enhanced recovery of gastrointestinal function,reduce complications,shorten the hospitalization time.
