摘 要:目的分析褪黑素对肺炎克雷伯菌感染大鼠炎症因子和核因子-Kappa B(NF-κB)表达的影响。方法32只雄性SD大鼠随机分为4组,各8只。对照组为健康大鼠,模型组采用气管滴入方法建立克雷伯杆菌感染肺炎大鼠模型,褪黑素组造模后给予2 mg/kg褪黑素灌胃,阳性对照组造模后予以10 mg/kg左氧氟沙星灌胃,统计各组大鼠一般情况、肺组织病理组织学变化、外周血检测白细胞(WBC)和中性粒细胞(PMN)计数、肺组织炎症因子[白细胞介素-1β(IL-1β)、IL-6、IL-8、肿瘤坏死因子-α(TNF-α)]、NF-κB mRNA、NF-κB抑制蛋白α(IκBα)mRNA。结果肺炎克雷伯菌接种后模型组出现呼吸喘鸣音明显,呼吸急促,面部四肢紫绀等肺炎症状,褪黑素组、阳性对照组症状较模型组轻,对照组试验过程进食、活动度无明显变化;肺组织组间表现比较相似,对照组未见明显异常,模型组肺泡上皮见退行性变化,肺泡管扩张,各级支气管周围、肺泡间隔结缔组织血管周围、小叶间炎性细胞大量浸润,病变严重区域连成片,肺泡壁血管充血,褪黑素组、阳性对照组较模型组明显减轻;模型组WBC、PMN计数高于对照组(P<0.05),褪黑素组和阳性对照组比较无显著差异,但均低于模型组(P<0.05);模型组IL-1β、IL-6、IL-8、TNF-α水平均高于对照组(P<0.05),褪黑素组、阳性对照组比较无显著差异,但均低于模型组(P<0.05);模型组NF-κB、IκBαmRNA高于对照组(P<0.05),褪黑素组、阳性对照组比较无显著差异,但均低于模型组(P<0.05)。结论褪黑素可通过抑制NF-κB信号通路发挥降低肺组织炎症因子水平,改善肺部炎症反应的作用,其效果与左氧氟沙星相似,可能为肺炎克雷伯菌感染的治疗提供了一个新思路。OBJECTIVE To explore the effect of melatonin on expression of inflammatory factors and NF-Kappa B(NF-κB)in rats with Klebsiella pneumoniae infection.METHODS Totally 32 male SD rats were randomly divided into 4 groups,with 8 rats in each group.The rats in the control group were healthy rats,the model group was treated with intratracheal drip to establish the model of rats with K.pneumoniae infection,the melatonin group was given 2 mg/kg of melatonin by gastric lavage,and the positive control group was given 10 mg/kg of levofloxacin by gastric lavage.The general condition,pathological changes of lung tissue,peripheral blood white blood cell(WBC)and neutrophil(PMN)counts,inflammatory factors in lung tissues[interleukin-1 beta(IL-1 beta),IL-6,IL-8,tumor necrosis factor-alpha(TNF-α)],NF-κB mRNA,and NF-κB inhibitory proteinα(IκBα)mRNA were statistically analyzed.RESULTS After the K.pneumoniae was inoculated,the rats in the model group had the pneumonia symptoms such as obvious respiratory stridor,shortness of breath,cyanosis of face and limbs,the symptoms of the melatonin group and the positive control group were milder than those of the model group,while there was no significant change in food intake and activity in the control group during the experiment.As compared with the manifestations of the lung tissues,there was no obvious abnormality in the lung tissue of the control group,the alveolar epithelium of the model group showed degenerative changes,alveolar tube dilatation,and bronchi,alveolar septal connective tissues and lobules infiltrated with a large number of inflammatory cells,patchy shadow in severe lesions area and congestion of alveolar wall blood vessels;the above symptoms of the melatonin group and the positive control group were significantly milder than those of the model group.The WBC and PMN counts of the model group were significantly higher than those of the control group(P<0.05),there were no significant differences between the melatonin group and the positive control group,however,the abo
