制霉素片联合复方氯己定含漱液对固定正畸矫正患者口腔念珠菌感染的临床疗效

摘　　要：目的探究制霉素片联合复方氯己定含漱液对固定正畸矫正患者口腔念珠菌感染的临床疗效及安全性。方法选取2017年3月-2018年3月于首都医科大学附属北京口腔医院王府井部进行固定正畸矫正合并口腔念珠菌感染患者92例,随机分为对照组(46例)和研究组(46例)。对照组采用复方氯己定含漱液进行治疗,研究组采用制霉素片联合复方氯己定含漱液进行治疗。两组患者均在治疗2周后通过对口腔真菌菌落数、口腔疼痛评分及安全性进行疗效评价。结果治疗2周后,研究组的总有效率为89.13%(41/46),显著高于对照组的71.74%(33/46)(P=0.036)。两组患者在治疗后,真菌菌落数、口腔疼痛评分较治疗前均降低(P<0.05),且研究组患者真菌菌落数(32.65±8.06)×10^3 CFU/ml、口腔疼痛评分(0.45±0.26)分均低于对照组患者(P<0.05)。在治疗期间,两组患者均出现黏膜轻度刺激感、味觉轻度改变和口腔色素沉着不良反应,但不良反应症状均较轻微,且停药2周后上述症状明显好转。结论制霉素片联合复方氯己定含漱液对固定正畸矫正患者口腔念珠菌感染具有良好的疗效,可明显降低患者的口腔疼痛及真菌菌落数,安全性较好,可在临床推广使用。OBJECTIVE To explore the clinical efficacy and safety of nystatin tablets combined with compound chlorhexidine gargle in treatment of fixed orthodontics patients with oral Candida infection.METHODS A total of 92 patients who were treated with fixed orthodontics and complicated with oral Candida infection in Wang Fujing Branch of Capital Medical University School of Stomatology from Mar.2017 to Mar.2018 were enrolled in the study and randomly divided into the control group with 46 cases and the study group with 46 cases.The control group was treated with compound chlorhexidine gargle,while the study group was given nystatin tablets combined with compound chlorhexidine gargle.The oral fungal colony counts,score of oral pain and safety were observed and compared between the two groups of patients after the treatment for 2 weeks so as to evaluate the curative effect.RESULTS The total effective rate of the study group was 89.13%(41/46)after the treatment for 2 weeks,significantly higher than 71.74%(33/46)of the control group(P=0.036).The fungal colony counts and score of oral pain of the two groups of patients were significantly lower after the treatment than before the treatment(P<0.05),the fungal colony counts of the study group were(32.65±8.06)×10^3 CFU/ml,significantly lower than those of the control group;the score of oral pain of the study group was(0.45±0.26)points,significantly lower than that of the control group(P<0.05).There were patients in both groups who had adverse reactions such as mild mucous membrane irritation,slight change in taste and oral pigmentation during the treatment period,all of which were remarkably improved after drug withdrawal for 2 weeks.CONCLUSION Nystatin tablets combined with compound chlorhexidine gargle can achieve favorable effect on treatment of the fixed orthodontics patients with oral Candida infection and significantly reduce the oral pain and fungal colony counts,and it is safe and worthy to be promoted in the hospital.
