消化道不同重建路径对胸腔镜联合食管癌手术治疗的影响比较

摘　　要：目的研究比较消化道不同重建路径对胸腔镜联合食管癌手术治疗的影响。方法选择2016年2月-2018年2月期间焦作市温县人民医院收治的88例胸腔镜联合食管癌手术患者作为研究对象,按照路径手术的不同,分为食管床组(n=45)、胸骨后路组(n=43),观察对比两组患者的手术指标以及并发症发生率。结果两组患者的手术时间、术中出血量、胃管留置时间差异无统计学意义(P>0.05),胸骨后路组的胃液引流量显著低于食管床组(P<0.05);两组患者的吻合狭窄、吻合口瘘、呼吸衰竭差异无统计学意义(P>0.05),胸骨后路组的肺部感染率显著低于食管床组(P<0.05)。结论消化道不同重建路径对患者胸腔镜联合食管癌手术治疗的影响差异不明显,临床上可以根据患者的病情、本人及家属要求以及经济情况等作出选择。Objective To study the effect of different reconstruction paths of digestive tract on thoracoscopy combined with esophageal cancer surgery.Methods 88 patients with thoracoscopic combined esophageal cancer surgery in the hospital from February,2016 to February,2018 were enrolled.Among them,45 patients underwent esophageal bed surgery(esophageal bed group),and 43 underwent sternal pathway surgery(posterior sternal group).The surgical index and the incidence of complications were compared between the two groups.Results There was no significant difference in the operation time,intraoperative blood loss and gastric tube indwelling time between the two groups(P>0.05).The gastric fluid drainage in the posterior sternal group was significantly lower than that in the esophageal bed group(P<0.05).The difference of anastomotic stenosis,anastomotic leakage and respiratory failure was not significant(P>0.05).The pulmonary infection rate of the posterior sternal group was significantly lower than that of the esophageal bed group(P<0.05).Conclusion The different reconstruction paths of the digestive tract have different effects on the surgical treatment of thoracoscopy combined with esophageal cancer.The clinical choices can be made according to the patient’s condition,the requirements of the family and their families,and the economic situation.
