摘 要:目的膀胱癌(bladder cancer,BC)的诊断通常采用侵入式膀胱镜检查,微小RNA(miRNA)在BC中表达失调,可以作为初级诊断和监测的非侵入性尿标记物。本研究旨在探讨尿液miRNA作为BC标记物的诊断价值。方法收集2017-12-01-2018-05-31在宁夏医科大学总医院泌尿外科就诊的48例膀胱癌、23例尿路感染及同期22名健康体检者的尿液,分为对照组、感染组、T1期肿瘤组、T2+T3期肿瘤组。采用Real time PCR技术,筛选尿液样本miRNA标志物;对筛选的miRNA以Ct值比作为计量方式,绘制ROC曲线分析其诊断价值。结果 Real time PCR结果显示,对照组miR-126、miR-182和miR-146a的Ct值分别为30.08±0.49、32.07±1.17和28.13±0.88,感染组分别为28.37±0.51、30.93±0.40和26.72±0.78,T1期肿瘤组分别为27.62±0.78、29.06±0.43和25.62±0.86,T2+T3期肿瘤组分别为26.04±0.74、25.61±0.64和22.7±0.75,差异均有统计学意义,P<0.05;miR-152表达稳定,组间差异均无统计学意义,P>0.05。ROC曲线分析发现,以miR-182与miR-152的Ct比值作为诊断标准,其诊断价值最高,AUC值为0.887 5,特异性80.00%,灵敏性83.33%。结论本研究建立的尿液miRNA比值模型,对于BC诊断有较高参考价值。OBJECTIVE Bladder cancer is usually diagnosed with invasive cystoscopy,and microRNA which is dysregulated can be used as a non-invasive urine marker for primary diagnosis and monitoring in BC.This article aimed to investigate the diagnostic value of urine miRNA as a tumor marker for bladder cancer.METHOD Collect the urine of 48 cases of bladder cancer,23 cases of urinary tract infections and 22 healthy medical examinees in the urology department of Ningxia Medical University General Hospital from December 2017 to May 2018.Divided into control group,infection group,T1 tumor group and T2+T3 tumor group.Real time PCR technology was used to screen the miRNA markers in urine,the ratio of Ct values of the selected miRNAs was used as the measurement method,the diagnostic method of the screened miRNA was established and verified,and the ROC curve was called to analyze its diagnostic value.RESULT Real time PCR results showed that the Ct values of miR-126,miR-182 and miR-146 awere in the control group(30.08±0.49,32.07±1.17,28.13±0.88),the infected group(28.37±0.51,30.93±0.40,26.72±0.78),T1 tumor Group(27.62±0.78,29.06±0.43,25.62±0.86)and T2+T3 tumor group(26.04±0.74,25.61±0.64,22.7±0.75),were statistically significant(P<0.05),and the expression of miR-152 was no statistical significance(P>0.05).Diagnostic value finding the ratio of miR-182 to miR-152 was the highest in diagnostic value,with an AUC value of 0.8875,specificity of 80.00%,and sensitivity of 83.33%.CONCLUSION The urine miRNA ratio model established in this study has a high reference value for the diagnosis of bladder cancer.
