牙髓再生术对Beagle犬牙体牙髓病模型中炎症微环境的作用

摘  要:目的探讨牙髓病变程度与炎性因子水平的关系以及牙髓再生术对犬牙体牙髓病炎性因子的影响。方法4个月龄雄性比格犬10只,随机分为根尖感染组和去牙髓组各5只(每组各20颗牙)。每组再根据手术方式分为牙髓再生术亚组和根尖诱导成形术亚组各10颗牙。4个亚组患牙均行X线检查,并行组织病理学检查HE染色,采用ELISA法检测牙髓P物质(substance P,SP)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-1(interleukin-1,IL-1)水平。结果造模4周时,根尖感染组和去牙髓组X线检查可见患牙根尖暗影;术后12周,4个亚组患牙根尖病变区范围均有不同程度减小或消失,根尖孔闭合,根尖感染组牙髓再生术亚组较根尖诱导成形术亚组牙髓组织炎症细胞明显减少;术后12周,4个亚组犬牙髓组织SP、VEGF、TNF-α和IL-1水平均低于术前(P<0.05);根尖感染组的根尖诱导成形术亚组牙髓组织SP[(33.28±2.39)ng/L]、VEGF[(41.35±9.35)ng/L]、TNF-α[(67.08±10.62)ng/L]和IL-1[(85.26±4.23)ng/L]水平低于牙髓再生术亚组[(82.12±2.66)、(98.18±7.82)、(124.62±9.20)、(120.75±5.40)ng/L](P<0.05);去牙髓组的牙髓再生术亚组牙髓组织SP[(48.64±3.90)ng/L]、VEGF[(56.38±4.46)ng/L]、TNF-α[(58.25±9.04)ng/L]和IL-1[(42.42±5.68)ng/L]水平低于根尖诱导成形术亚组[(88.05±3.56)、(110.20±6.90)、(96.38±7.85)、(90.28±6.40)ng/L](P<0.05);去牙髓组的牙髓再生术亚组牙髓组织SP、VEGF、TNF-α和IL-1水平低于根尖感染组的牙髓再生术亚组(P<0.05),根尖诱导成形术亚组牙髓组织SP、VEGF、TNF-α和IL-1水平高于根尖感染组的根尖诱导成形术亚组(P<0.05)。结论牙髓病变程度与牙髓组织炎症细胞因子IL-1、TNF-α、SP和VEGF水平具有一定相关性,牙髓再生术和根尖诱导成形术均可修复牙髓病变,但牙髓再生术对去牙髓患牙的治疗效果更佳。Objective To investigate the relationship between the degree of pulp lesion and the level of inflammatory cytokines,as well as the effect of pulp regeneration on the inflammatory cytokines of canine dental pulp disease.Methods Ten 4-month-old male beagles were equally and randomly divided into apical infection group and pulpectomy group,with 20 teeth in each group.According to the treatment,each group was redivided into pulp regeneration subgroup and apical induction subgroup,with 10 teeth in each group.These four subgroups underwent X-ray examination and HE staining.The levels of substance P(SP),vascular endothelial growth factor(VEGF),tumor necrosis factor-α(TNF-α)and interleukin-1(IL-1)were detected by ELISA.Results The apical shadow was found in two groups in 4 weeks of modeling.The apical lesion area shrank or disappeared and the apical foramen was closed in 4 subgroups after 12 weeks.The inflammatory cells in the pulp tissue were less in pulp regeneration subgroup than in apical induction subgroup.The levels of SP,VEGF,TNF-αand IL-1 in four subgroups were lower in 12 weeks after operation than those before operation(P<0.05).In apical infection group,the levels of SP((33.28±2.39)ng/L),VEGF((41.35±9.35)ng/L),TNF-α((67.08±10.62)ng/L)and IL-1((85.26±4.23)ng/L)in apical induction subgroup were lower than those in pulp regeneration subgroup((82.12±2.66),(98.18±7.82),(124.62±9.20),(120.75±5.40)ng/L)(P<0.05).In pulpectomy group,the levels of SP((48.64±3.90)ng/L),VEGF((56.38±4.46)ng/L),TNF-α((58.25±9.04)ng/L)and IL-1((42.42±5.68)ng/L)in pulp regeneration subgroup were lower than those in apical induction subgroup((88.05±3.56),(110.20±6.90),(96.38±7.85),(90.28±6.40)ng/L)(P<0.05).The levels of SP,VEGF,TNF-αand IL-1 in pulp regeneration subgroup were lower in pulpectomy group than those in apical infection group(P<0.05),while the levels of SP,VEGF,TNF-αand IL-1 in apical induction group were higher in pulpectomy group than those in apical infection group(P<0.05).Conclusion The degree of pulp lesi
