摘　　要：目的探讨N-钙黏蛋白(N-cadherin)在胆管癌组织中的表达及意义。方法选取2012年1月至2019年6年在西安医学院第二附属医院和第四军医大学西京医院治疗并经病理科确诊进行手术切除的80例胆管癌患者,提取癌组织及≥2 cm的癌旁组织的总RNA,用实时荧光定量PCR(RT-PCR)法检测胆管癌组织及癌旁组织N-cadherin的mRNA,分析胆管癌组织/癌旁组织中N-cadherin的mRNA比值与临床病理参数间的联系,并根据N-cadherin mRNA在胆管癌组织和癌旁组织的表达差异绘制做出ROC曲线,分析N-cadherin mRNA对胆管癌的诊断价值。结果与癌旁组织比较,胆管癌组织中N-cadherin mRNA升高;N-cadherin mRNA在胆管癌患者性别、不同年龄间的表达无统计学差异(P>0.05),与胆管癌患者的分化程度、TNM分期、淋巴结转移及神经浸润有关,差异具有统计学意义(P<0.05)。ROC曲线结果显示:胆管癌组织/癌旁组织的N-cadherin mRNA的AUC面积为0.88,诊断敏感性为83.75%,特异性为77.50%。结论胆管癌组织中N-cadherin mRNA高于癌旁组织,与患者肿瘤的分化成程度、TNM分期及淋巴结转移相关,且对胆管癌具有诊断价值,其表达可能对胆管癌病情的发生发展具有重要的作用。Objective To investigate the expression and significance of N-cadherin in cholangiocarcinoma.Methods Total RNA was extracted from 80 patients of cholangiocarcinoma treated in our hospital and confirmed by pathology from January 2012 to June 2016.The expression of N-cadherin in cholangiocarcinoma and adjacent tissues was detected by RT-PCR.The ratio of N-cadherin in cholangiocarcinoma and adjacent tissues was analyzed.The relationship between pathological parameters and ROC curve were drawn according to the expression difference of N-cadherin in cholangiocarcinoma and adjacent tissues.The diagnostic value of N-cadherin in cholangiocarcinoma was analyzed.Results Compared with the adjacent tissues,the expression of N-cadherin was increased in cholangiocarcinoma tissues,and there were no statistical significantly differences in sex and age(P>0.05),which was related to the differentiation degree,TNM stage,lymph node metastasis and neural infiltration of cholangiocarcinoma patients,and there were statistical significantly differences(P<0.05).ROC curve showed that the area of AUC of N-cadherin mRNA in cholangiocarcinoma/paracancerous tissues was 0.88,the diagnostic sensitivity was 83.75%,and the specificity was 77.50%.Conclusion The expression of N-cadherin in cholangiocarcinoma is higher than that in adjacent tissues,which is related to the degree of differentiation,TNM stage and lymph node metastasis of the tumors,and has diagnostic value for cholangiocarcinoma.The expression of N-cadherin may play an important role in the occurrence and development of cholangiocarcinoma.
