尿管球囊扩张联合早期呼吸训练治疗脑卒中后吞咽功能障碍的疗效

摘　　要：目的:探讨尿管球囊扩张联合早期呼吸训练对脑卒中后吞咽功能障碍患者的疗效。方法:选择我院收治的脑卒中伴吞咽障碍患者96例,随机分为呼吸训练组(48例)和联合训练组(48例)。两组患者在接受常规临床药物治疗及综合吞咽康复治疗外,还接受早期的呼吸训练。联合训练组患者在此基础上还接受尿管球囊扩张环咽肌训练。采用吞咽障碍特异性生活质量量表(SWALQOL)评价患者生活质量,治疗4周后比较两组患者的生活质量及康复疗效,于出院时记录两组患者留置胃管时间和吸入性肺炎的发生情况。结果:与呼吸训练组比较,治疗后联合训练组患者留置胃管时间[(16.34±2.14)d比(11.24±2.02)d]显著缩短,生活质量评分[(138.44±21.16)分比(159.73±20.85)分]及康复总有效率(79.17%比95.24%)均显著提高,P<0.05或<0.01。结论:尿管球囊扩张联合早期呼吸训练对脑卒中后吞咽障碍患者具有较好的康复效果,值得推广。Objective:To explore therapeutic effect of urinary catheter balloon dilation combined early respiratory training on dysphagia after stroke.Methods:A total of 96 patients with dysphagia after stroke treated in our hospital were randomly and equally divided to respiratory training group and combined training group.Both groups received early respiratory training besides routine medication and comprehensive swallowing rehabilitative therapy.Combined training group received additional urinary catheter balloon dilation training on pharynx.Quality of life(QOL)was assessed by swallow quality of life questionnaire(SWALQOL),QOL and rehabilitation therapeutic effect were compared between two groups after four-week treatment,indwelling gastric tube time and incidence of aspiration pneumonia were recorded in two groups at discharge.Results:After treatment,compared with respiratory training group,there was significant reduction in indwelling gastric tube time[(16.34±2.14)dvs.(11.24±2.02)d],and significant rise in SWALQOL score[(138.44±21.16)scores vs.(159.73±20.85)scores]and total effective rate of rehabilitation(79.17%vs.95.24%)in combined training group,P<0.05 or<0.01.Conclusion:Urinary catheter balloon dilation combined early respiratory training possesses significant rehabilitative effect on patients with dysphagia after stroke,which is worth extending.
