摘　　要：目的分析H3K79me2在结直肠癌组织中的表达水平,并探讨其与患者临床病理特征及预后的关系。方法选取2012年5月至2014年12月苏州市立医院本部/南京医科大学附属苏州医院确诊为结直肠癌的78例手术病例为研究对象,通过免疫组化检测H3K79me2在结直肠癌组织中的表达,分析H3K79me2表达水平与患者临床病理特征及预后的关系,并探讨其与外周血癌胚抗原(CEA)水平的相关性。结果H3K79me2在结直肠癌组织中的表达高于癌旁正常组织(P<0.01);结直肠癌组织中H3K79me2表达水平与年龄、性别、肿瘤位置、分化程度、有无脉管侵犯及M分期无关(均P>0.05),与N分期、T分期、TNM分期相关(均P<0.05);T分期和N分期是H3K79me2高表达的独立危险因素(均P<0.05);结直肠癌组织中H3K79me2的表达与外周血CEA水平呈正相关(r=0.474,P<0.01);随访87个月,生存分析提示结直肠癌患者中H3K79me2高表达组的生存率低于低表达组(P=0.01),外周血CEA升高组患者生存率低于正常组(P<0.05);生存预后的多因素分析提示H3K79me2表达、CEA水平是结直肠癌患者预后的独立影响因素(P<0.05)。结论结H3K79me2的表达与结直肠癌的发生、发展关系密切,可能有助于临床患者的病情判断及预后评估,并为结直肠癌的生物治疗提供新的可能靶点。Objectives To investigate the level of expression of H3K79me2 in colorectal cancer tissues and its relationship with clinicopathological features and prognosis.Methods Seventy-eight patients with confirmed diagnosis of colorectal cancer who underwent surgery at Suzhou Municipal Hospital/Suzhou Affiliated Hospital of Nanjing Medical University between May 2012 and December 2014 were recruited.The expression of H3K79me2 in colorectal cancer tissues was analyzed using immunohisto⁃chemistry.We analyzed its correlation with clinicopathological features,prognosis,and peripheral blood level of carcinoembryonic antigen(CEA).Results Expression of H3K79me2 was significantly higher in the colorectal cancer tissue than in the paracancer⁃ous tissue(P<0.01).The level of H3K79me2 expression in the colorectal cancer tissue was independent of age,sex,location of tumor,degree of differentiation,vascular invasion,and M staging(P>0.05),but was significantly associated with N,T,and TNM staging(P<0.05).Independent explanatory factors for H3K79me2 expression were T and N staging(P<0.05).The level of H3K79me2 expression in the colorectal cancer tissue also positively correlated with CEA levels(r=0.474,P<0.01).Patients were followed up for 87 months.Survival rate was significantly lower in patients with high H3K79me2 expression than in those with low expression(P=0.01).Patients with high levels of CEA also had significantly lower survival rate than patients with low levels of CEA(P<0.05).Multivariate survival analyses showed that H3K79me2 expression and CEA level were independent risk factors for poor survival(P<0.05).Conclusion H3K79me2 expression is associated with the development and progression of colorectal cancer.It could be useful for clinical evaluation and prediction of prognosis.It could also be used as a new target for develop⁃ment of novel biological treatments for colorectal cancer.
