摘 要:目的:探讨动态脑电图(AEEG)在评估重症脑卒中昏迷患者脑功能损害程度及预后中的价值。方法:回顾性分析2017年1月至2019年12月湖南省脑科医院神经内科NICU诊治的患者50例,所有患者行格拉斯哥昏迷量表(GCS)评分及AEEG监测。以昏迷发生1个月时或患者死亡为判定终点,参照Pittsburgh脑功能分级标准,将终点评定分为良好转归、不良转归及临床死亡。比较3种不同转归患者GCS评分的差异,并将AEEG分级与临床预后及GCS评分进行相关性分析。结果:50例患者中,良好转归15例,不良转归18例,临床死亡17例。3种不同转归患者的GCS评分差异均有统计学意义(P<0.05);AEEG异常分级与临床预后呈中度正相关(r=0.677,P<0.05),AEEG异常分级与GCS评分呈中度负相关(r=-0.524,P<0.05)。结论:AEEG监测对于评估重症脑卒中昏迷患者脑功能及预后转归有重要意义,值得临床推广。Objective:To explore the value of ambulatory electroencephalography(AEEG)in evaluating the degree of brain function damage and prognosis in comatose patients with severe stroke.Methods:Fifty patients diagnosed and treated in neurology intensive care unit of Brain Hospital of Hunan Province from January 2017 to December 2019 were analyzed retrospectively.According to Pittsburgh brain function grading standard,the subjects were divided into good outcome group,bad outcome group and clinical death group.The degree of coma in the three groups was evaluated by Glasgow coma scale(GCS),and the brain function was evaluated by AEEG monitoring.The differences of GCS were compared among the three groups,and the correlation analysis of AEEG grade with clinical prognosis and GCS score was performed.Results:Among the 50 patients,15 cases had good outcome,18 cases had bad outcome and 17 cases died.There were significant differences for GCS score among the three groups(P<0.05).AEEG abnormal grades were positively correlated with clinical prognosis(r=0.677,P<0.05),while AEEG abnormal grades were negatively correlated with GCS score(r=-0.524,P<0.05).Conclusion:AEEG monitoring is of great significance in evaluating the brain function and prognosis of comatose patients with severe stroke.
