乙型肝炎病毒大蛋白检测在慢性乙型肝炎诊疗中的临床意义

摘　　要：目的:探讨乙型肝炎病毒大蛋白(HBV LP)检测在慢性乙型肝炎(CHB)诊疗中的临床意义。方法:选取2017年1月至2018年1月CHB患者89例,采用酶联免疫吸附法法检测HBV LP表达,采用实时荧光定量PCR检测HBV DNA水平。结果:CHB患者HBV LP阳性率为89.89%,明显高于HBV DNA阳性率(P<0.05);HBV DNA拷贝数对数值>7的患者HBV LP吸光度值为(1.522±0.322),明显高于其他患者(P<0.05);HBV LP吸光度值与HBV DNA拷贝数对数值呈正相关(r=0.846,P<0.05);治疗24周时,HBV DNA和HBV LP阳性率比较差异无统计学意义(P>0.05);治疗48周时,HBV LP阳性率为48.57%,明显高于HBV DNA阳性率(P<0.05)。结论:HBV LP检测在CHB诊疗过程中有重要应用价值;在不能开展HBV DNA检测的机构,可用HBV LP作为替代检测;当HBV DNA阴性或低水平时,可将HBV LP作为抗病毒治疗终点判定的指标。Objective:To investigate the clinical significance of detection of hepatitis B virus large protein(HBV LP)in the diagnosis and treatment of chronic hepatitis B.Methods:89 cases of chronic hepatitis B treated in our hospital from January 2017 to January 2018 were selected,the expression of HBV LP was detected by enzyme-linked immunosorbent assay,and the level of HBV DNA was detected by real-time quantitative PCR.Result:The positive rate of HBV LP in patients with chronic hepatitis B was 89.89%,which was significantly higher than that of HBV DNA(P<0.05);HBV DNA copy number logarithm value>7 patients HBV LP absorbance value was(1.522±0.322),which was significantly higher than other patients(P<0.05);HBV LP absorbance value was positively correlated with HBV DNA copy number logarithm value(r=0.846,P<0.05);At 24 weeks treatment,there was no significant difference in the positive rate of HBV DNA and HBV LP(P>0.05);At 48 weeks treatment,the positive rate of HBV LP was 48.57%,which was significantly higher than that of HBV DNA(P<0.05).Conclusion:HBV LP detection has important application value in the diagnosis and treatment of chronic hepatitis B;In the absence of HBV DNA testing institutions,HBV LP can be used to replicate HBV replication;When HBV DNA is negative or low,HBV LP can be used as an indicator of the end point of antiviral therapy.
