摘  要:目的研究三阴性乳腺癌患者超声血流分级及其与孕激素受体(PR)、雌激素受体(ER)、抑癌基因p53(p53)表达的相关性,为临床三阴性乳腺癌的诊断提供依据。方法回顾性分析2015年1月至2018年12月在安康市中医医院就诊且经过手术治疗的84例三阴性乳腺癌患者的临床资料。比较所有患者的超声征像,分析其免疫组织化学检查结果,并研究三阴性乳腺癌患者的血流分级与ER、PR、p53表达的相关性。结果 84例三阴性乳腺癌患者中48例(57.14%) ER表达呈阳性,48例(57.14%) PR表达呈阳性,44例(52.38%) ER和PR表达均呈阳性,24例(28.57%) p53表达呈阳性;存在"毛刺征"患者中ER、PR、p53表达阳性的患者明显高于ER、PR、p53表达阴性患者,差异均有统计学意义(P<0.05);肿瘤血流分级0~Ⅰ级患者中ER、PR、p53表达阳性患者明显多于ER、PR、p53表达阴性患者,差异均有统计学意义(P<0.05);肿瘤血流Ⅱ~Ⅲ分级患者中p53表达阳性患者明显多于p53表达阴性患者,差异有统计学意义(P<0.05)。结论三阴性乳腺癌患者的血流分级与ER、PR、p53表达有相关性,ER、PR、p53水平的检测对三阴性乳腺癌患者的诊断、治疗与预防有着较为重要的意义。Objective To study the relationship between ultrasound blood flow classification and the expression of progesterone receptor(PR),estrogen receptor(ER),and tumor suppressor gene p53(p53)in three negative breast cancer patients,to provide basis for clinical diagnosis of triple negative breast cancer.Methods The clinical data of 84 cases of triple negative breast cancer who were treated in Ankang Traditional Chinese Medicine Hospital from January 2015 to December 2018 were analyzed retrospectively.The ultrasonic images of all patients were compared,and the results of immunohistochemistry were analyzed.The correlation between the blood flow classification and the expression of ER,PR,p53 in three negative breast cancer patients were studied.Results In 84 cases of triple negative breast cancer,ER expression was positive in 48 cases(57.14%),PR expression was positive in 48 cases(57.14%),ER and PR expression was positive in 44 cases(52.38%),and p53 expression was positive in 24 cases(28.57%).The positive expression of ER,PR,and p53 in patients with“burr sign”was higher than that in patients with negative expression of ER,PR,and p53,and the difference was statistically significant(P<0.05).The positive expression of ER,PR,and p53 in patients with tumor blood flow grade 0-Ⅰwas significantly higher than that in patients with negative expression of ER,PR and p53,and the differences were statistically significant(P<0.05).In patients with tumor blood flow gradesⅡtoⅢ,the number of p53 positive patients was significantly more than that of p53 negative patients,and the difference was statistically significant(P<0.05).Conclusion The grading of blood flow in patients with triple negative breast cancer is related to the expression of ER,PR,and p53,which is of great significance for the diagnosis,treatment and prevention of triple negative breast cancer.
