三瓣吻合矫正术与提上睑肌缩短+翼状韧带悬吊术治疗重度上睑下垂患者的疗效比较

摘　　要：目的研究三瓣吻合矫正术与提上睑肌缩短+翼状韧带悬吊术治疗重度上睑下垂的效果。方法选取我院重度上睑下垂患者96例(2018年5月~2019年5月),按照随机数字表法分为A组、B组,各48例。其中A组采用三瓣吻合矫正术治疗,B组采用提上睑肌缩短+翼状韧带悬吊术治疗。比较两组疗效、术前、术后3个月眼睑生理功能(上睑活动度、眼睑闭合时睑裂高度)及美学效果评分。结果A组总有效率95.83%高于B组83.33%(P<0.05);术后3个月A组上睑活动度大于B组,眼睑闭合时睑裂高度小于B组(P<0.05);术后3个月A组美学效果评分高于B组(P<0.05)。结论与提上睑肌缩短+翼状韧带悬吊术比较,三瓣吻合矫正术治疗重度上睑下垂效果显著,能改善眼睑生理功能,提高美学效果。Objective To study the effect of three-valve anastomosis and levator palpebral muscle shortening+pterygoid ligament suspension on severe ptosis.Methods 96 patients with severe blepharoptosis in our hospital(May 2018 to May 2019)were selected and divided into groups A and B according to the random number table method,each with 48 cases.Among them,group A was treated with three-valve anastomosis,and group B was treated with levator eyelid muscle shortening+pterygoid ligament suspension.The curative effect,preoperative and postoperative 3 months of eyelid physiological function(upper eyelid mobility,eyelid fissure height when eyelid closed)and aesthetic effect score were compared between the two groups.Results The total effective rate in group A was 95.83%higher than that in group B(P<0.05);3 months after operation,the upper eyelid activity of group A was greater than that of group B,and the height of the eyelid fissure when the eyelid was closed was less than that of group B(P<0.05);three months after surgery,the aesthetic effect score of group A was higher than that of group B(P<0.05).Conclusion Compared with levator eyelid muscle shortening+pterygoid ligament suspension,three-valve anastomosis has significant effect on severe blepharoptosis,which can improve the physiological function of eyelid and aesthetic effect.
