摘 要:[目的]分析山西省2012—2017年两个周期宫颈癌检查项目的筛查结果,评价项目实施情况及工作质量。[方法]收集山西省2012—2014年(第2周期)与2015—2017年(第3周期)115个项目县(区)、筛查人数2587229人次的宫颈癌筛查项目数据;采用χ^2检验比较第2周期和第3周期不同宫颈癌初筛方法阳性病例检出情况,以及进一步检查后宫颈癌前病变、宫颈癌等检出情况。[结果]在第2、3周期项目实施中,运用宫颈细胞学进行初筛的比例逐年上升,由第2周期的30.54%上升到第3周期的65.40%;细胞学初筛阳性率提高,由第2周期的4.2%上升到第3周期的4.8%;阴道镜检查异常率提高,由第2周期的39.64%上升到第3周期的44.33%;组织病理学检查异常率提高,由第2周期的18.99%上升到第3周期的32.11%;χ^2值分别为1455.13、278.23、1154.15(P均<0.001)。宫颈癌前病变检出率由第2周期的156.44/10万增至第3周期的283.15/10万,宫颈癌检出率由第2周期的47.83/10万增至第3周期的54.52/10万,早期诊断率由第2周期的83.83%增至第3周期的87.63%。[结论]山西省宫颈癌筛查方案在2012—2017年2个周期中逐步优化,筛查的有效性、准确性明显提升。宫颈癌前病变、宫颈癌的检出率及早期诊断率逐年提高。[Purpose]To analyze the screening results of cervical cancer examination items in two cycles from 2012 to 2017,and evaluate the project implementation and work quality.[Methods]The cervical cancer screening project data of 115 project counties(districts)and 2587229 screening people in 2012-2014(cycle 2)and 2015-2017(cycle 3)in Shanxi Province was collected.χ^2 test was used to compare the positive cases detected by different primary screening methods in the second and third cycles of cervical cancer,as well as the detection of cervical precancerous lesions and cervical cancer in further examination.[Results]In the implementation of the second-third cycle project,the proportion of cervical cytology for primary screening had increased year by year,from 30.54%in the second cycle to 65.40%in the third cycle.The positive rate of primary cytology screening increased from 4.20%in the second cycle to 4.8%in the third cycle.The abnormal rate of colposcopy increased from 39.64%in the second cycle to 44.33%in the third cycle;The abnormal rate of histopathological examination increased from 18.99%in the second cycle to 32.11%in the third cycle.χ^2 was 1455.13,278.23,1154.15,all P<0.001.The detection rate of cervical precancerous lesions increased from 156.44/105 in the second cycle to 283.15/105 in the third cycle,the detection rate of cervical cancer increased from 47.83/105 in the second cycle to 54.52/105 in the third cycle,and the early diagnosis rate increased from 83.83%in the second cycle to 87.63%in the third cycle.[Conclusion]The cervical cancer screening program of Shanxi Province is gradually optimized in two six-year cycles from 2012 to 2017,and the effectiveness and accuracy of the screening are significantly improved.The detection rate and early diagnosis rate of cervical precancerous lesions and cervical cancer are increasing year by year.
