乙型肝炎病毒DNA检测与电化学免疫发光分析技术结果的相关性研究

摘　　要：目的探讨乙型肝炎病毒(HBV)DNA检测与电化学免疫发光分析(ECLIA)技术结果的相关性。方法选择168例HBV感染者为研究对象,采用荧光定量聚合酶链反应法(FQ-PCR)测定其HBV-DNA含量;ECLIA技术测定血清乙肝e抗原(HBeAg)、乙肝表面抗原(HBsAg)、乙肝e抗体(HBeAb)、乙肝表面抗体(HBsAb)、乙肝核心抗体(HbcAb)。分析两种测定结果的相关性。结果 HBV-DNA检测结果显示,168例患者中,HBV-DNA阳性率为52.38%;ECLIA测定结果中,HBeAg、HBsAg、HBe Ab、HBsAb、HbcAb阳性率分别为22.62%、67.26%、47.02%、39.29%、77.98%。38例大三阳患者HBV-DNA含量和阳性率均显著高于72例小三阳者(P<0.05)。Pearson相关性分析结果显示,患者血清HBsAg、HBeAb、HBsAb、HbcAb与HBV-NDA含量呈正相关(r=0.803、0.745、0.579、0.626,P<0.01)。Kappa一致性检验结果显示,血清HBeAb测定结果与HBV-NDA检测具有较好一致性,HBsAg、HBsAb、HbcAb与HBV-NDA一致性中等。结论在HBV感染检测中,采用ECLIA技术测定血清标志物HBeAb的诊断结果与HBV-NDA结果有较好一致性,可推荐应用于HBV感染快速诊断中。Objective To investigate the correlation between hepatitis B virus(HBV)DNA detection and electrochemical immunoluminescence analysis(ECLIA)results.Methods A total of 168 patients with HBV infection were selected as the research objects.The HBV-DNA level was determined by fluorescence quantitative-polymerase chain reaction(FQ-PCR);the serum hepatitis e antigen(HBeAg),hepatitis E surface antigen(HBsAg),hepatitis B e antibody(HBeAb),hepatitis B surface antibody(HBsAb)and hepatitis B core antibody(HbcAb)were determined by ECLIA.The correlation between the two results was analyzed.Results HBV-DNA test results showed that the HBV-DNA positive rate was 52.38%in 168 patients;the results of ECLIA showed that the positive rates of HBeAg,HBsAg,HBeAb,HBsAb and HBcAb were 22.62%,67.26%,47.02%,39.29% and 77.98%respectively.The HBV-DNA level and positive rate of 38 patients with positive HBeAg and HBsAg were significantly higher than those of 72 patients with positive HBsAg(P<0.05).Pearson correlation analysis showed that there were a positive correlation between serum HBsAg,HBeAb,HBsAb,HbcAb and HBV-DNA levels(r=0.803、0.745、0.579、0.626,P<0.01).Kappa consistency test results showed that serum HBeAb showed good consistency with HBV-NDA test,HBsAg,HBsAb and HbcAb showed moderate consistency with HBV-NDA.Conclusion In the detection of HBV infection,the diagnostic results of serum markers HBeAb using ECLIA technology are in good agreement with the results of HBV-NDA,and can be recommended for rapid diagnosis of HBV infection.
