自体血清在难治性角膜溃疡患者结膜掩盖术后的应用效果探究

摘　　要：目的探讨自体血清在难治性角膜溃疡患者结膜掩盖术后的应用效果。方法选择2018年1月~2018年12月我院(目前在陕西支援,病例来自于两个医院)接诊的86例难治性角膜溃疡,通过随机数表法分为观察组46例和对照组40例,对照组给予结膜掩盖术治疗,术后给予常规护理,观察组在结膜掩盖术后联合自体血清治疗,连续用药30d。比较两组临床疗效、临床症状缓解时间、视力及并发症。结果治疗后3个月后,观察组临床疗效总有效率明显比对照组高(P<0.05);观察组畏光流泪、异物感、角膜溃疡、前房脓肿缓解时间明显短于对照组(P<0.05);治疗后3个月后,观察组视力<0.02的比例明显低于对照组(P<0.05);两组继发青光眼、角膜葡萄肿、角膜穿孔、结膜瓣溶解总发生率比较差异无统计学意义(P>0.05)。结论自体血清在难治性角膜溃疡患者结膜掩盖术后的应用效果满意,可有效促进角膜修复,改善视力,临床应用价值高。Objective To study the curative efficacy of autologous serum in patients with refractory corneal ulcer after conjunctival masking.Methods 86 patients of refractory corneal ulcer who received therapy from January 2018 to December 2018 in our hospital were selected as research objects,according to the random number table,those patients were divided into 46 cases in the observation group and 40 cases in the control group,the control group was treated with conjunctival masking and routine nursing after operation,the observation group was treated with autologous serum after conjunctival masking,the drug was administered continuously for 30 days.The clinical efficacy,remission time of clinical symptoms,visual acuity and complications were compared between the two groups.Results After treatment 3 months,the total effective rate in the observation group was significantly higher than that of the control group(P<0.05);the relief time of photophobia,tears,foreign body sensation,corneal ulcer and anterior chamber abscess in the observation group was significantly shorter than that of the control group(P<0.05);after treatment 3 months,the proportion of visual acuity<0.02 in the observation group was significantly lower than that of the control group(P<0.05);there was no significant difference in the total incidence of secondary glaucoma,corneal staphyloma,corneal perforation and conjunctival valve lysis between the two groups(P>0.05).Conclusion Autologous serum is well for patients with refractory corneal ulcer after conjunctival masking,which can effectively promote corneal repair and improve visual acuity,it has high clinical value.
