摘　　要：[目的]探讨轻度胃肠炎伴惊厥(CwG)的婴幼儿外周血炎性因子水平及临床意义。[方法]选取2018年2月~2018年12月我院确诊为CwG的56例婴幼儿作为观察组,20例健康婴幼儿作为对照组。对比2组患儿的外周血TNF-α、IL-6、IL-10表达水平,分析CwG患儿在不同惊厥发作次数和不同惊厥发作时间的外周血TNF-α、IL-6、IL-10表达水平,通过ROC曲线分析TNF-α、IL-6、IL-10指标在CwG婴幼儿中的诊断效能。[结果]观察组患儿血清TNF-α、IL-6、IL-10表达水平明显高于对照组,差异有统计学意义(P<0.05);惊厥发作次数≥2次的患儿血清TNF-α、IL-6、IL-10表达水平显著高于惊厥发作次数<2次的患儿,惊厥发作时间≥5 min的患儿血清TNF-α、IL-6、IL-10表达水平显著高于惊厥发作时间<5 min的患儿,差异有统计学意义(P<0.05);ROC曲线分析及最大约登指数计算出血清TNF-α、IL-6、IL-10指标最大AUC面积的截止值分别为:TNF-α截断值为1.71 pg/mL(灵敏度=81.70%,特异度=92.20%),IL-6截断值为5.47 pg/mL(灵敏度=69.80%,特异度=84.60%),IL-10截断值为5.84 pg/mL(灵敏度=77.50%,特异度=91.20%)。[结论]CwG婴幼儿外周血TNF-α、IL-6、IL-10水平呈高表达,其水平越高,预示着发生惊厥次数越多,发生惊厥时间越长。临床上监测CwG婴幼儿外周血TNF-α、IL-6、IL-10水平变化,可辅助临床诊断婴幼儿发生惊厥的情况。[Objective] To investigate the level of inflammatory factors in peripheral blood of infants with mild gastroenteritis and convulsions and its clinical significance.[Methods]From February 2018 to December 2018,56 infants with mild gastroenteritis and convulsions were selected as observation group and 20 healthy infants as control group.The expression levels of TNF-α,IL-6 and IL-10 in peripheral blood of two groups were compared.The expression levels of TNF-α,IL-6 and IL-10 in peripheral blood of children with CwG at different seizure times and different seizure time were analyzed.The diagnostic efficacy of TNF-α,IL-6 and IL-10 in children with CwG was analyzed by ROC curve.[Results]The levels of serum TNF-α,IL-6 and IL-10 in the observation group were significantly higher than those in the control group(P<0.05);the levels of serum TNF-α,IL-6 and IL-10 in children with convulsions more than twice were significantly higher than those in children with convulsions less than twice,and the time of convulsions more than 5 minutes.The expression level of TNF-α was significantly higher than that of children with convulsion less than 5 minutes,and the difference was statistically significant(P<0.05).The cut-off values of TNF-α,IL-6 and IL-10 were 1.71 pg/mL(sensitivity=81.70%,specificity=92.20%)and IL-6 were calculated by ROC curve analysis and maximum Yoden index.The values were 5.47 pg/mL(sensitivity=69.80%,specificity=84.60%)and 5.84 pg/mL(sensitivity=77.50%,specificity=91.20%).[Conclusion]The levels of TNF-α,IL-6 and IL-10 in peripheral blood of infants with mild gastroenteritis and convulsions were highly expressed.The higher the level,the more convulsions occurred and the longer the convulsions occurred.Clinical monitoring of TNF-α,IL-6 and IL-10 levels in peripheral blood of infants with mild gastroenteritis and convulsions can assist in clinical diagnosis of convulsions in infants.
