摘　　要：[目的]研究真核细胞起始因子4 E(eukaryotic initiation factor 4 E,eIF4E)、C-myc及细胞周期蛋白依赖性激酶4(cyclin-dependent kinase 4,CDK4)在胃癌及胃癌前情况中的表达及意义。[方法]收集胃黏膜活检组织及外科手术切除标本共202例,包括慢性非萎缩性胃炎23例,慢性萎缩性胃炎30例,胃黏膜低级别上皮内瘤变25例,胃黏膜高级别上皮内瘤变27例,胃癌97例(同时收集距肿瘤边缘2~3 cm的癌旁组织),用免疫组化法测定上述标本中eIF4E、C-myc及CDK4的表达。[结果]慢性非萎缩性胃炎患者胃黏膜中均未检出eIF4E蛋白阳性表达;eIF4E蛋白阳性表达率从慢性萎缩性胃炎组及低级别上皮内瘤变组→高低级别上皮内瘤变组→胃癌组逐渐上升(P<0.01),且胃癌组中有淋巴结转移者、进展期者eIF4E蛋白阳性表达率在分别显著高于无淋巴结转移者、早期者(P<0.01)。在胃癌癌旁黏膜组织eIF4E蛋白阳性表达率明显高于其非癌对应黏膜组织(P<0.01或P<0.05);而且癌旁与相应癌灶eIF4E蛋白检测结果存在显著正相关(r=0.343,P<0.01)。C-myc和CDK4在慢性非萎缩性胃炎中均未见阳性表达,在胃癌前情况及胃癌中其阳性表达率逐渐升高(P<0.01);胃癌组与胃癌前情况各组、高级别上皮内瘤变组与慢性萎缩性胃炎组及低级别上皮内瘤变组相比差异亦有显著性(P<0.01或P<0.05)。胃癌组织中eIF4E分别与C-myc及CDK4呈非常显著正相关(r分别为0.652、0.494,P<0.01),而胃癌前情况各组中eIF4E分别与C-myc及CDK4亦呈非常显著正相关(r分别为0.775、0.694,P<0.01)。[结论]追踪胃癌前情况患者胃黏膜eIF4E蛋白表达可能有助于提高胃癌的早期诊断率;胃癌及胃癌前情况中C-myc及CDK4表达水平增高可能与eIF4E高表达有关,为深入研究胃癌的发病机制提供了线索。[Objective]To study the expression and significance of eukaryotic initiation factor 4E(eIF4E),C-myc and cyclin-dependent kinase 4(CDK4)in the diagnosis of gastric carcinoma and premalignant conditions.[Methods]A total of 202gastric mucosal biopsies and surgical resection specimens were collected from March 2016to December 2017in Wuhan Puren Hospital,including 23cases of chronic nonatrophic gastritis,30cases of chronic atrophic gastritis,25cases of gastric mucosal low-grade intraepithelial neoplasia,27cases of gastric mucosal high-grade intraepithelial neoplasia and 97cases of gastric carcinoma(including adjacent tissues of 2-3cm from the edge of the carcinoma).The expression of eIF4E,C-myc and CDK4in the above specimens were detected by immunohistochemistry.[Results]There was no positive expression of eIF4Eprotein in gastric mucosa of patients with chronic non-atrophic gastritis.In premalignant conditions of gastric carcinoma and gastric carcinoma,the expression of eIF4Ewas significantly different,showing an increasing trend(P<0.01).Furthermore,Lymph node metastasis group and advanced group were significantly higher than those without lymph node metastasis group and early group(P<0.01).The positive expression rate of eIF4Eprotein in gastric carcinoma paracancerous atrophic gastritis group,paracancerous low-grade intraepithelial neoplasia group and high-grade adjacent intraepithelial neoplasia group was significantly higher than that of non-cancerous corresponding group(P<0.01or P<0.05).In addition,there was a significant positive correlation between the detection of eIF4Eprotein in the adjacent to carcinoma and the corresponding carcinoma(r=0.343,P<0.01).There was no positive expression of C-myc and CDK4in gastric mucosa of patients with chronic non-atrophic gastritis,and the positive expression rate increased gradually in premalignant conditions of gastric carcinoma and gastric carcinoma.The positive expression rates of C-myc and CDK4in premalignant conditions of gastric carcinoma group and gastric carcinoma gro
