摘  要:目的探讨溃疡性结肠炎(UC)患者肠道菌群、外周血单个核细胞(PBMC)中白细胞介素-32(IL⁃32)、微小RNA⁃223(miR⁃223)、微小RNA⁃505(miR⁃505)水平与预后相关性。方法选取2017年1月-2019年4月佛山市高明区人民医院收治的92例UC患者作为研究组,并对患者进行规范化治疗,选取同期体检健康者100例作为对照组,观察两组研究对象及不同严重程度患者肠道菌群、PBMC中IL⁃32、miR⁃223、miR⁃505水平变化,肠道菌群、PBMC中IL⁃32、miR⁃223、miR⁃505表达相关性及与患者预后的关系。结果研究组大肠杆菌、乳酸菌属、双歧杆菌属数量、IL⁃32、miR⁃223表达均高于对照组,miR⁃505表达低于对照组,差异均有统计学意义(P<0.05);重度组、中度组患者大肠杆菌、乳酸菌属、双歧杆菌属数量与PBMC中IL⁃32、miR⁃223表达高于轻度组,重度组高于中度组,差异均有统计学意义(P<0.05),重度组患者PBMC中miR⁃505表达低于中度组、轻度组(P<0.05);UC患者大肠杆菌、乳酸菌属、双歧杆菌属均数与IL⁃32、miR⁃223表达呈正相关,与miR⁃505表达呈负相关,差异均有统计学意义(P<0.05),miR⁃505与IL⁃32、miR⁃223表达呈负相关,IL⁃32与miR⁃223表达呈正相关,差异均有统计学意义(P<0.05);预后不良组患者大肠杆菌、乳酸菌属、双歧杆菌属数量与PBMC中IL⁃32、miR⁃223表达高于预后良好组,miR⁃505表达低于预后良好组,差异均有统计学意义(P<0.05)。结论UC患者肠道菌群失调,PBMC中IL⁃32、miR⁃223高表达,miR⁃505低表达,肠道菌群失调及IL⁃32、miR⁃223、miR⁃505表达与患者预后相关。Objective To investigate the correlations between intestinal flora,interleukin⁃32(IL⁃32),microRNA⁃223(miR⁃223),microRNA⁃505(miR⁃505)levels in peripheral blood mononuclear cells(PBMC)and prognosis in patients with ulcerative colitis(UC).Methods 92 UC patients admitted to our hospital from January 2017 to April 2019 were selected as the research objects,and standardized treatment was given to the patients.Another 100 healthy persons in the same period were selected as the control group.The changes of intestinal flora,IL⁃32,miR⁃223 and miR⁃505 levels in PBMC,the correlations between intestinal flora,the expressions of IL⁃32,miR⁃223 and miR⁃505 in PBMC,and their relationships with prognosis of the subjects and patients with different severity were observed.Results The numbers of Escherichia coli,Lactobacillus and Bifidobacterium,the expressions of IL⁃32 and miR⁃223 in the study group were significantly higher than those in the control group(P<0.05),and the expression of miR⁃505 was significantly lower than that in the control group(P<0.05).The numbers of Escherichia coli,Lactobacillus and Bifidobacterium,the expressions of IL⁃32 and miR⁃223 in PBMC in severe group and moderate group were significantly higher than those in mild group(P<0.05);the severe group was higher than the moderate group(P<0.05),and the expression of miR⁃505 in PBMC in severe group was lower than that in moderate group and mild group(P<0.05).Means of E.coli,Lactobacillus and Bifidobacterium in UC patients were positively correlated with the expressions of IL⁃32 and miR⁃223(P<0.05),and negatively correlated with the expression of miR⁃505(P<0.05);the miR⁃505 was negatively correlated with expressions of IL⁃32 and miR⁃223(P<0.05),while IL⁃32 was positively correlated with the expression of miR⁃223(P<0.05).The numbers of Escherichia coli,Lactobacillus,Bifidobacterium and the expressions of IL⁃32 and miR⁃223 in PBMC of patients in poor prognosis group were significantly higher than those of patie
