摘  要:目的探讨CT与MRI多模态影像融合结合3D打印技术在大脑镰旁脑膜瘤切除术中的应用效果。方法收集诊治的5例大脑镰旁脑膜瘤患者临床资料,将凭借CT、MRI等多模态医学影像检查获取的影像数据重建形成三维虚拟模型,再结合3D打印技术打印肿瘤及其周围组织血管等实物模型。观察肿瘤实体解剖模型的构建并评价肿瘤切除程度,进一步观察患者术前、术后神经功能变化。结果5例患者均成功构建三维虚拟模型,并制作出大脑镰旁脑膜瘤实体解剖模型,清晰显示肿瘤与周围组织血管的关系;手术医师利用该模型进行术前模拟、制订手术方案及指导术中操作,提高了肿瘤切除率,避免术中并发症。结论多模态影像融合结合3D打印技术可为大脑镰旁脑膜瘤外科手术提供全面的导航信息,提高了肿瘤切除程度及手术安全性,有助于患者术后快速康复。Objective To investigate the effect of multi-modality combined with 3D printing technique on the resection of meningiomas in the cerebral palsy.Methods Retrospectively study the clinical data of 5 patients which were admitted to our department from with cerebral palsy meningioma.A three-dimensional visual composite virtual model is formed based on CT and MRI of all patients.A physical model which shows the location of brain tumor as well as its surrounding blood vessels is printed based on 3D printing technology.Evaluate the extent of tumor resection and observe the patients’neurological function before and after operation for further study.Results All five patients were successfully constructed a three-dimensional visual composite virtual model and produced a solid anatomical model of the paraspinal meningioma,which clearly shows the relationship between the tumor and its surrounding tissue vessels.Surgeons perform preoperative simulation using the model printed,make surgical plans and guide intraoperative operation.The whole procedure improves tumor resection rate and avoids intraoperative complications.4 patients achieved neurological improvement,1 patient occur neurological decline whose KPS scores were over 60 which fulfilled daily living.Conclusion Multimodal image fusion combined with 3D printing technology can provide comprehensive navigation information for cerebral cerebral palsy meningioma resection surgery,improves tumor resection rate and surgical safety and accieve rapid recovery after the resection surgery.
