摘 要:目的了解患有糖尿病但尚未发生糖尿病视网膜病变(DR)的患者黄斑各分区视网膜厚度改变。方法实验组选取我院内分泌科已经确诊糖尿病患者40例,对照组选取门诊体检无糖尿病者34例。采用Optovue RTVue的光谱域OCT对上述2组受试者进行双眼黄斑部扫描,结果分区并逐一分析各区全层、内层及外层视网膜厚度。结果糖尿病组内层视网膜旁中心凹区、中心凹周围区上方较无糖尿病组薄,且差异显著。糖尿病组外层视网膜中心凹周围区鼻侧较无糖尿病组薄,且差异显著。糖尿病组全层视网膜黄斑各区普遍较无糖尿病组薄,在黄斑中心凹、中心凹周围区颞侧和下方,差异无统计学意义(P>0.05)。其余黄斑各区厚度均有显著性差异。结论糖尿病无明显视网膜病变患者中,黄斑区视网膜厚度的改变受血供影响较大,在相对缺血的区域视网膜较薄。糖尿病黄斑区水肿和黄斑区视网膜变薄可能是同一疾病病程中的两个不同阶段。Objective To investigate the changes of retinal thickness in macular area in patients with diabetes but without diabetic retinopathy.Methods The experimental group selected 40 diabetic patients diagnosed by the endocrinology department of our hospital,while the control group selected 34 non-diabetic patients.The macular of the two groups were scanned with the spectral domain OCT of OptovueRTVue.The scanning results were divided,and then the thickness of the whole layer,inner layer and outer layer of the retina in each zone was analyzed.Results The parafovea area and the superior-perifovea area of the inner retina in the diabetic group was thinner than that in the non-diabetic group,and the difference was significant.The nasal-perifovea area of the outer retinal in the diabetic group was thinner than that in the non-diabetic group,and the difference was significant.The macular regions of the full-thickness retina in the diabetic group were generally thinner than those in the non-diabetic group.But,the differences were not significant(P>0.05)in the fovea area,tempo-perifovea area and inferior-perifovea area.There were significant differences in the thickness of other areas of the macula.Conclusion In diabetic patients without diabetic retinopathy,the change of macular retinal thickness was greatly affected by retinal blood supply,and the retina was thinner in the relatively ischemic area.Macular edema and thinning of macular retina in diabetic patients may be two different stages in the progression of the same disease.
