摘 要:目的观察正常人和可疑原发性房角关闭(PACS)、原发性房角关闭(PAC)和原发性闭角型青光眼(PACG)患者在饮水试验(WDT)后眼前节形态的动态变化。方法本研究包括30名健康受试者,16名PACS,10名PAC和6名PACG患者。在WDT之前和之后30 min,分别通过iCare回弹式眼压计和A型超声检查来测量眼压(IOP)和眼轴。用旋转scheimpflug相机测量前房容积(ACV)、前房角度(ACA)、周围前房深度(PACD)、中央前房深度(CACD)和瞳孔直径(PD)。结果WDT前后青光眼患者眼压[(18.5±2.4)至(27.4±3.6)mmHg]、健康受试者眼压[(12.9±2.1)至(17.3±2.4)mmHg]均明显升高(P<0.01)。患者和对照组的IOP升高幅度存在明显差异[(9.0±2.3)vs.(4.4±1.6)mmHg,P<0.05]。尽管两组在WDT后ACV和PACD均明显降低,但组间差异并不明显。此外,饮水试验对CACD、PD及眼轴的影响不明显。结论饮水试验可减小健康受试者及闭角型青光眼患者的ACV和PACD,引起前房动态变化。青光眼患者饮水试验后眼压升高更为明显。饮水试验可作为ACS、PAC、PACG的激发试验。Objective:To observe the dynamic changes in the anterior segment morphology after the water drinking test(WDT)in normal subjects and patients with primary angle closure suspect(PACS),primary angle closure(PAC),and primary angle-closure glaucoma(PACG).Methods:This study included 30 healthy participants,16 PACS,10 PAC,and 6 PACG patients.30 min prior to and after the WDT,the intraocular pressure(IOP)and axis were measured by iCare rebound tonometry and A-type ultrasonography,respectively.The anterior chamber volume(ACV),anterior chamber angle(ACA),peripheral anterior chamber depth(PACD),central anterior chamber depth(CACD)and pupil diameter(PD)were measured with a rotating scheimpflug camera.Results:The IOP was significantly elevated after the WDT in patients[(18.5±2.4)to(27.4±3.6)]mmHg and healthy participants[(12.9±2.1)to(17.3±2.4)mmHg](P<0.01).The IOP range differed significantly between patients and controls[(9.0±2.3)vs.(4.4±1.6)mmHg;P<0.05].Although the ACV and PACD decreased significantly after the WDT in both groups,differences between-group were not significant,and neither group exhibited significant changes in the CACD,PD,or axis.Conclusion:Water drinking causes dynamic changes in the anterior chamber by decreasing the ACV and PACD in both healthy participants and patients with PACS,PAC,PACG.The increase in the IOP is more obvious in patients.The WDT may be used as a provocation test for PACS,PAC and PACG.
