3种不同根管消毒药物对窦道型根尖周炎的疗效研究

摘　　要：目的评价3种根管消毒药物氢氧化钙糊剂、氢氧化钙碘仿糊剂及三联抗生素糊剂TAP(奥硝唑、米诺环素和环丙沙星)对窦道型根尖周炎的疗效。方法选择240例窦道型根尖周炎的患者,共240例患牙,根据根管消毒药物的不同,随机分成3组,分别为氢氧化钙糊剂组,氢氧化钙碘仿糊剂组和三联抗生素糊剂组,每组80例患牙。患牙常规根管预备后,将3种根管消毒药物置入根管内,同时采用视觉模拟评分法。分别于封药后1,2、3周并嘱患者根管封药1周内自行记录视觉模拟疼痛量表(VAS),观察封药后疼痛反应、根管消毒效果及根管治疗预后的效果。结果氢氧化钙糊剂组与氢氧化钙碘仿糊剂组、氢氧化钙糊剂组与三联抗生素糊剂组在封药1周、2周后疗效差异有统计学意义(P<0.05),封药3周后疗效无统计学意义。氢氧化钙碘仿糊剂组与三联抗生素糊剂组在1、2及3周的疗效差异无统计学意义(P>0.05)。三组的总有效率之间无统计学意义(P>0.05)。VAS评分结果显示,在首次封药后的前3天内,三联抗生素糊剂组与氢氧化钙碘仿糊剂组在疼痛控制方面均明显优于氢氧化钙糊剂组,差异有统计学意义(P<0.05)。结论氢氧化钙糊剂,氢氧化钙碘仿糊剂及三联抗生素糊剂对窦道型根尖周炎均有良好的临床疗效,氢氧化钙碘仿糊剂和三联抗生素糊剂较氢氧化钙糊剂能缩短根管治疗的时间,且氢氧化钙糊剂和三联抗生素糊剂对根管治疗后疼痛的控制较为有效。Objective To compare the efficacy in disinfection and pain control of calcium hydroxide paste,calcium hydroxide iodoform paste and triple antibiotic paste TAP(ornidazole,ciprofloxacin and minocycline)used for root canal disinfection of periapical abscess with sinuses.Methods Two hundred and forty patients with chronic apical periodontitis combined with sinus(240 teeth)were involved in the present study and divided into 3 groups:calcium hydroxide paste group,calcium hydroxide iodoform paste group and TAP group,each group has 80 teeth.After regular root canal preparation,patients in 3 groups were filled with 3 different drugs respectively.Visual analogue scales(VAS)of pain were given to the patients with a guide for filling the scale.One week later,the data of the scales were recorded and analyzed and the effects of root canal disinfection were recorded and analyzed in one week,two weeks and three weeks.Results There was statistically significant difference at the clinical efficacy between calcium hydroxide paste group and calcium hydroxide iodoform paste group after treatment in one week,two weeks(P<0.05).and after 3 weeks there was not significant difference between the two groups(P>0.05).in the same way between calcium hydroxide paste group and TAP group.The clinical efficacy of calcium hydroxide paste group,calcium hydroxide iodoform paste group and TAP group was similar after treatment in in one week,two weeks and three weeks(P>0.05).VAS score analysis showed that,at least on the first 3 days after sealing medicine in the root canal,TAP group and calcium hydroxide iodoform paste group achieved better result of pain control than calcium hydroxide paste group(P<0.05).Conclusion Calcium hydroxide paste,calcium hydroxide iodoform paste and TAP are effective in treatment of chronic apical periodontitis with sinuses.TAP group and calcium hydroxide iodoform paste used in root canal disinfection can decrease treatment time on chronic apical periodontitis with sinuses the same as pain control.
