摘　　要：目的:观察肿瘤标志物神经元特异性烯醇化酶(NSE)、骨胶素-细胞角蛋白19片段(Cyfra21-1)、癌胚抗原(CEA)与非小细胞肺癌(NSCLC)胸膜转移的相关性。方法:选取2018年1月-2019年1月NSCLC患者40例(NSCLC组)、良性肺疾病患者40例(BLD组)及健康人40例(健康组)。对比分析三组受检者血清中NSE、CYFRA21-1、CEA水平。结果:NSCLC组血清NSE、CYFRA21-1、CEA水平明显高于BLD组及健康组,差异有统计学意义(P<0.05)。以健康组为对照,对NSCLC诊断灵敏度分别为NSE为29.5%、CYFRA21-1为63.9%、CEA为47.2%。以BLD组为对照组,对NSCLC诊断灵敏度分别为NSE为16.8%、CYFRA21-1为48.5%、CEA为39.8%。对鳞癌诊断灵敏度最高的是CYFRA21-1为66.1%,对腺癌诊断灵敏度最高的是CEA为53.6%,NSE在NSCLC胸膜转移的阳性率为14.0%~20.5%。结论:在对NSCLC胸膜转移诊断中,CYFRA21-1诊断鳞癌具有较高的价值;CEA诊断腺癌具有较高的优势,NSE诊断价值较小。Objective:To observe the correlation between tumor markers such as NSE,CYFRA21-1,CEA and pleural metastasis of non-small cell lung cancer.Methods:From January 2018 to January 2019,40 patients with non-small cell lung cancer(NSCLC group),40 patients with benign lung disease(BLD group)and 40 healthy patients(healthy group)were selected.The concentrations of serum NSE,CYFRA21-1,CEA were compared and analyzed in three groups.Results:The serum NSE,CYFRA21-1,CEA levels of NSCLC group were significantly higher than the BLD group and healthy group,the differences were statistically significant(P<0.05).Compared with the healthy group,the diagnostic sensitivity for NSCLC was 29.5%of NSE,63.9%of CYFRA21-1 and 47.2%of CEA,respectively.Compared with BLD group,the diagnostic sensitivity for NSCLC was NSE of 16.8%,CYFRA21-1 of 48.5%and CEA of 39.8%,respectively.The highest diagnostic sensitivity for squamous cell carcinoma was CYFRA21-1(66.1%),the highest diagnostic sensitivity for adenocarcinoma was CEA(53.6%),the positive rate of NSE in pleural metastasis of non-small cell lung cancer was 14.0%to 20.5%.Conclusion:In the diagnosis of pleural metastasis of non-small cell lung cancer,CYFRA21-1 has high value in the diagnosis of squamous cell carcinoma,CEA has a higher advantage in the diagnosis of adenocarcinoma and the value of NSE is less.
