摘　　要：目的:评价甲基纤维素联合前房注气术在减轻硬核白内障超生乳化手术后角膜水肿的效果。方法:采用前瞻性队列研究方法,选取92例(114只眼)Ⅳ级核以上年龄相关性白内障患者作为研究对象,随机分为研究组(甲基纤维素联合前房注气)和对照组(仅用透明质酸钠)。实验组在水分离完成后在角膜后注入甲基纤维素,并且水密前房后通过侧切口前房注入0.2mL过滤空气,对照组采用常规手术方法(透明质酸钠作为黏弹剂)。手术前后测量两组角膜内皮细胞数及角膜厚度来观察角膜水肿情况。结果:两组术后均出现部分角膜水肿,药物干预后恢复透明。研究组术后平均角膜厚度与对照组术比较有显著的统计学意义(P<0.01);两组术后角膜内皮计数有显著的统计学意义(P<0.01)。结论:甲基纤维素与前房注气术联合应用能明显减轻硬核白内障超声乳化术后的角膜水肿情况。Objective:To investigate the effect of methyl-cellulose combined with anterior chamber gas injec-tion on corneal edema after supergrowth emulsification for hard nucleus cataract.Methods:Prospective cohort study method,choose 92 cases(114 eyes)Ⅳgrade and above as the research object,age-related cataract patients were randomly divided into research group and the control group.After water separation,methyl cellulose was injec-ted into the back of the cornea,and 0.2 mL filtered air was injected into the front chamber through a side incision after the watertight front chamber.Sodium hyaluronate was used as a viscoelastic agent in the control group for rou-tine surgery.Corneal endothelial cells and corneal thickness were measured before and after the operation,and cor-neal edema was observed.Results:Partial corneal edema was observed in both groups,and the transparency was restored after drug intervention.The difference of average postoperative corneal thickness between the study group and the control group was statistically significant(P<0.01);The postoperative corneal endothelial count in the two groups was statistically significant(P<0.01).Conclusion:The combined application of methyl cellulose and anterior chamber gas injection can significantly reduce the corneal edema after phacoemulsification for hard nucleus cataract.
