口腔鳞状细胞癌患者中膜型基质金属蛋白酶1、基质金属蛋白酶2的表达水平及临床意义

摘　　要：目的探讨口腔鳞状细胞癌患者中膜型基质金属蛋白酶1(MT1-MMP)、基质金属蛋白酶2(MMP2)的表达水平及临床意义。方法选择83例接受根治性手术治疗的口腔鳞状细胞癌患者,采用免疫组织化学染色方法检测口腔鳞状细胞癌组织中MT1-MMP、MMP2的表达情况,分析其与患者临床特征及生存情况的关系,采用Spearman相关分析法分析MT 1-MMP、MMP2表达的相关性。结果TNM分期为Ⅲ~Ⅳ期、中-低分化、有淋巴结转移、复发的口腔鳞状细胞癌患者口腔鳞状细胞癌组织中MT1-MMP和MMP2的强阳性表达率分别高于TNM分期为Ⅰ~Ⅱ期、高分化、无淋巴结转移、未复发的口腔鳞状细胞癌患者,差异均有统计学意义(P<0.05)。不同性别、年龄、肿瘤部位、浸润情况口腔鳞状细胞癌患者口腔鳞状细胞癌组织中MT1-MMP和MMP2的强阳性表达率比较,差异均无统计学意义(P>0.05)。口腔鳞状细胞癌患者中MT1-MMP与MMP2的表达呈正相关(P<0.05)。MT1-MMP强阳性表达患者的中位生存时间为24个月,明显短于MT1-MMP弱阳性/阴性表达患者的32个月,差异有统计学意义(P<0.01)。MMP2强阳性表达患者的中位生存时间为24个月,明显短于MMP2弱阳性/阴性表达患者的33个月,差异有统计学意义(P<0.01)。结论口腔鳞状细胞癌患者中MT1-MMP、MMP2的表达情况与TNM分期、分化程度、淋巴结转移、复发及预后有关,强阳性表达患者的预后更差。Objective To investigate the expression of membrane type 1-matrix metalloproteinase(MT1-MMP) and matrix metalloproteinase 2(MMP2) in patients with oral squamous cell carcinoma(OSCC) and the clinical significance.Method Eighty-three patients of OSCC who underwent radical surgery were included in the analysis,among them,the expression of MT1-MMP and MMP2 were detected by immunohistochemical staining,to analyze the relationship with patient’s clinical characteristics and survival.Spearman correlation analysis was used to determine the correlation between MT1-MMP and MMP2.Result The strong positive rates of MT1-MMP and MMP2 in patients with TNM stage Ⅲ-Ⅳ,moderate or poor differentiation,positive lymph node metastasis and recurrent OSCC were significantly increased compared to those with TNM stage Ⅰ-Ⅱ,high differentiation,negative lymph node metastasis and no recurrent OSCC,the differences were statistically significant(P<0.05).There was no significant difference in the expression levels of MT1-MMP and MMP2 among patients with different gender,age,tumor location,and infiltration(P>0.05).A significantly positive correlation between MT1-MMP and MMP2 expression were noted in OSCC patients(P<0.05).The median survival time of patients with strong positive MT1-MMP expression was 24 months,which was notably shorter than the 32 months in those with negative/weak positive expression of MT1-MMP,and the difference was of statistical significance(P<0.01).The median survival time of patients with strong positive MMP2 expression was 24 months,which was notably shorter than the 33 months in those with negative/weak positive expression of MMP2,and the difference was of statistical significance(P<0.01).Conclusion The expression levels of MT1-MMP and MMP2 are related to TNM stage,differentiation,metastasis,recurrence and prognosis among patients with OSCC,their strong positive expression indicates an even worse prognosis.
