肿瘤自身抗体与肿瘤标记物在肺癌诊断中的意义

摘　　要：目的探讨7项肿瘤自身抗体(CAGE、GAGE7、GBU4-5、MAGE A1、P53、PGP9.5、SOX2)与4项肿瘤标记物(CEA、CYFRA21-1、NSE、SCCA)在肺癌诊断中的意义。方法收集宁波市李惠利医院2018年5月-11月362例就诊者,其中非肺部疾病患者50例,肺部良性疾病患者145例,健康体检者59例,肺癌108例,采用ELISA法分别检测各组的自身抗体和血清肿瘤标记物,绘制ROC工作曲线图,分析其对肺癌的诊断效能,并比较7种抗体在各组间的阳性率。结果肺癌组7项自身抗体水平,显著高于其余3组,差异有统计学意义(P=0.000)。7项肿瘤自身抗体联合检测肺癌敏感度为35.19%,特异度为92.13%,AUC为0.785,其敏感度和AUC均高于单项检测,4项肿瘤标志物联合检测AUC为0.647,低于7项自身抗体的联合检测。结论7项自身抗体联合检测优于肿瘤标记物联合检测,若联合两者,可作为肺癌的早期筛查手段。Objective To investigate the diagnostic value of seven tumor autoantibodies including CAGE,GAGE7,GBU4-5,MAGE A1,P53,PGP9.5,SOX2 and four tumor markets including CEA,CYFRA21-1,NSE,SCCA in the lung cancer.Methods To collect 362 cases of patients with lung cancer recruited in Ningbo Medical Center(Group)Li Huili Hospital from May to November 2018,including 50 cases of non pulmonary diseases,145 cases of benign lung diseases,59 cases for physical examinations,and 108 cases of lung cancer;their autoantibodies and serum tumor markets were tested by enzyme linked immunosorbent assay(ELISA),ROC working curve was drawn and used to analyze their diagnostic efficiency for lung cancer and compare positive rate of seven autoantibodies between different groups.Results The levels of seven autoantibodies in the lung cancer group were significantly higher than those in the other three groups,and the difference was statistically significant(P=0.000).The sensitivity,specificity and AUCROC of the combination of seven autoantibodies in the preliminary diagnosis of lung cancer were 35.19%,92.13%,0.785,respectively,and its sensitivity and AUCROC were higher than those of single autoantibody.The area of AUCROC by combined detection of four tumor markers was 0.647,which was lower than that by the combined detection of seven autoantibodies.Conclusion The combined detection of seven autoantibodies is superior to the combined detection of tumor markers.If combined with the two,it can be used as an early screening method for lung cancer.
