摘  要:目的探讨涎腺乳腺样分泌性癌(MASC)病例临床病理学特点、免疫表型、鉴别诊断及分子遗传学特征。方法收集4例涎腺MASC,总结该组病变的临床资料,并通过苏木精-伊红(HE)染色、免疫组织化学及荧光原位杂交的方法观察该组病变的病理及分子遗传特征。结果4例MASC患者年龄32~55岁,中位年龄42岁,其中1例为实体型,其余3例均有显著的囊性变。MASC主要组织学特点为肿瘤细胞形成微囊或滤泡样结构,其内含有PAS染色阳性的液体。肿瘤弥漫表达S100及Vimentin,ETV6基因分裂检测均阳性。结论涎腺MASC是一种罕见的组织学亚型,其组织学形态、免疫表型、分子生物学特征及临床生物学行为均与MASC相似。S100蛋白弥漫表达及ETV6基因的分离,可作为诊断及鉴别诊断的重要参考。Objective To investigate the clinicopathological characteristics,immunophenotypes,differential diagnosis and molecular genetics features of mammary analogue secretory carcinoma(MASC)of salivary glands.Methods Four cases with MASC of salivary glands were collected,and the clinical data of this group were summarized.The pathological and molecular genetic characteristics of the lesions were observed by HE staining,immunohistochemistry and fluorescence in situ hybridization.Results Four cases with MASC were 32-55 years old,with an average age of 42 years.One case was solid and the other 3 cases had significant cystic degeneration.The main histological feature of MASC was that tumor cells formed microcapsules or follicular like structures,which contained periodic acid-schiff stain(PAS)positive staining liquid.Diffuse expression of S100 and Vimentin were found in tumor which was of positive detection by ETV6 gene split.Conclusion The MASC of salivary glands is a rare histologic subtype.The histology,immunophenotype,molecular biological characteristics and clinical biological behavior of the salivary gland are similar to those of the secretory carcinoma of the breast.The diffuse expression of S100 protein and the isolation of ETV6 gene can be used as an important reference for diagnosis and differential diagnosis.
