经皮引导下甲状腺良性结节微波消融的疗效分析

摘　　要：目的探讨经皮超声引导下甲状腺良性结节消融治疗的临床疗效及安全性。方法对2015年5月至2019年4月江苏省肿瘤医院、淮安市洪泽区人民医院、南京市江宁医院、句容市人民医院及南通第一人民医院甲状腺治疗中心甲状腺良性结节980例,共1480枚病灶,超声实时监测及引导下将水冷式微波天线植入结节内,采用单点或多点一次性完全消融。消融后即时超声造影评价疗效。结果经过1个月、3个月、6个月、12个月随访,甲状腺结节体积及最大径持续缩小(P<0.05),12个月以后缩小不明显(P>0.05),共有124枚病灶消失。手术前后患者甲状腺功能无明显变化,无并发症发生。结论超声引导下甲状腺良性结节微波消融治疗安全有效、副作用小,值得临床推广。Objective To investigate the clinical efficacy and safety of percutaneous ultrasound-guided microwave ablation of benign thyroid nodules.Methods From May 2015 to April 2019,1480 thyroid benign nodules from 980 caseswere ultrasonically monitored and guided by a water-cooled microwave antenna tocomplete the ablationusing a single or multiple point one-time.Assessment of efficacy by contrast-enhanced ultrasound was performed immediately after ablation.Results During the follow-up of 12 months,all of the nodules had been completely necrotic after ablation whose volume and the largest diameter shrank gradually(P<0.05).There was no significant change in thyroid nodule size after 12 months(P>0.05).One hundred and twenty-four nodules disappeared.There was no significant change in thyroid function before and after surgery,and no complications occurred.Conclusions Ultrasound-guided microwave ablation of benign thyroid nodules is safe and effective with a low rate of complication,which is worthy of promotion.
