摘 要:目的探讨银杏达莫联合针灸通过核因子E2相关因子2(nuclear factor-E2-related factor 2,Nrf2)/血红素加氧酶-1(heme oxygenase-1,HO-1)通路促进大鼠创伤性脑出血(traumatic intracerebral hemorrhage,TICH)后神经功能恢复的作用机制研究。方法10只SD大鼠作为对照组,45只SD大鼠通过击打的方法建立TICH模型,建模成功后随机分为3组,即TICH组、银杏达莫组和银杏达莫联合针灸组,每组取10只。银杏达莫组大鼠腹腔注射银杏达莫,银杏达莫联合针灸组大鼠在银杏达莫组基础上进行针灸干预。对照组进行假手术和生理盐水干预。核磁共振成像检查脑出血情况,并根据Longa评分法评估神经功能缺损情况,使用水迷宫视频跟踪分析系统分析大鼠神经功能,TUNEL染色检测脑组织神经细胞凋亡情况,RT-qPCR和Western Blotting检测Nrf2和HO-1 mRNA和蛋白水平。结果核磁共振检查发现建模后小鼠脑组织出现出血情况。干预后银杏达莫组和银杏达莫联合针灸组Longa评分显著低于TICH组,且银杏达莫联合针灸组低于银杏达莫组(P<0.05)。与对照组比较,TICH组逃避潜伏期显著升高,而穿越平台次数显著降低(P<0.05);银杏达莫组和银杏达莫联合针灸组的逃避潜伏期显著低于TICH组,而穿越平台次数显著高于TICH组,银杏达莫联合针灸组的逃避潜伏期显著低于银杏达莫组,而穿越平台次数显著高于银杏达莫组(P<0.05)。与对照组比较,TICH组的脑组织神经细胞凋亡率显著升高,银杏达莫组和银杏达莫联合针灸组低于TICH组,且银杏达莫联合针灸组低于银杏达莫组(P<0.05)。与对照组比较,TICH组大鼠脑组织中Nrf2和HO-1mRNA和蛋白水平显著升高,银杏达莫组和银杏达莫联合针灸组高于TICH组,且银杏达莫联合针灸组高于银杏达莫组(P<0.05)。结论银杏达莫可与针灸协同作用,促进脑组织中Nrf2/HO-1通路的表达,从而调节氧化应激反应,保护TICH大鼠的神经功能。Objective To explore the mechanism of Yinxingdamo combined with acupuncture to promote neurological recovery after traumatic intracerebral hemorrhage(TICH)in rats through nuclear factor-E2-related factor 2(Nrf2)/heme oxygenase-1(HO-1)pathway.Methods Ten SD rats were used as a control group.Forty-five TICH model of SD rats were established by the method of hitting.After successful modeling,the rats were randomly divided into 3 groups:TICH group,Yinxingdamo group and Yinxingdamo combined with acupuncture group,with 10 rats in each group.Rats in the Yinxingdamo group were injected intraperitoneally with Yinxingdamo,and those in the Yinxingdamo combined with acupuncture group underwent acupuncture intervention on the basis of the Yinxingdamo group.The rats in the control group rats underwent sham surgery and saline intervention.Magnetic resonance imaging(MRI)was used to detect cerebral hemorrhage,and neurological deficits were assessed according to the Longa score.Neurological function of rats was analyzed using a water maze video tracking analysis system.TUNEL staining was used to detect apoptosis of cranial nerves.Nrf2,and HO-1 mRNA and protein levels were detected by RT-qPCR and Western blotting,respectively.Results MRI revealed bleeding in the brain tissue after modeling.Longa scores in the Yinxingdamo group and the Yinxingdamo combined with acupuncture group were significantly lower than those in the TICH group,and the Longa score of Yinxingdamo combined with acupuncture group was significantly lower than that of Yinxingdamo group(P<0.05).Compared with the control group,the escape latency of the TICH group was significantly increased and the number of crossing platforms was significantly decreased(P<0.05).The escape latency of the Yinxingdamo group and the Yinxingdamo combined with acupuncture group was significantly lower than that of the TICH group,and the number of crossing platforms was significantly higher than that of the TICH group,and the escape latency of the Yinxingdamo combined with acupuncture group
