摘　　要：目的研究DNA聚合酶δ3(POLD3)在肝细胞癌(HCC)癌组织及癌旁组织的表达水平及与临床病理的关系。方法选取2015年12月-2018年12月拟行手术治疗的HCC患者97例为研究对象,术中收集癌组织和癌旁组织标本,先进行HE染色复核病理诊断和分期,然后进行免疫组化染色,采用免疫组化反应评分(IRS)评估癌组织和癌旁组织POLD3表达水平,并分析癌组织POLD3表达与HCC临床病理特点和预后的关系。结果POLD3阳性定位于细胞核,颜色自浅棕色至棕黑色不等,癌组织POLD3高表达率低于癌旁组织(P<0.05);POLD3低表达的HCC患者肿瘤单发数量、直径、TNM分期及合并门静脉癌栓比例均高于高表达者,分化程度低于高表达者(P<0.05);术后随访5年显示POLD3高表达者和低表达者生存时间比较差异无统计学意义(P>0.05)。结论POLD3在HCC患者癌组织中表达水平降低,可能在HCC发病和进展过程中发挥重要作用,为HCC诊断、治疗和预后评估提供了新的思路和靶点。Objective To study the expression level of DNA polymeraseδ3(POLD3)in hepatocellular carcinoma(HCC)tissues and adjacent tissues and its relationship with clinicopathology.Methods A total of 97 HCC patients who were scheduled to undergo surgery in our hospital from December 2015 to December 2018 were selected for the study.The specimens of cancer tissues and adjacent tissues were collected during operation.The pathological diagnosis and staging of HE staining were performed initially,followed by immunohistochemical staining.Immunohistochemical reaction score(IRS)was used to evaluate the expression level of POLD3 in cancer tissues and adjacent tissues,and the relationship between expression of POLD3 in cancer tissues and clinicopathological features and prognosis of HCC was analyzed.Results POLD3 was positively located in the nucleus with color ranging from light brown to brownish black,and the high expression rate of POLD3 in cancer tissues was lower than that in adjacent tissues(P<0.05).The number of single tumor,diameter,TNM staging and proportion of combined portal vein tumor thrombus in HCC patients with low POLD3 expression were higher than those in patients with high expression while the differentiation degree was lower than that in patients with high expression(P<0.05).The 5-year follow-up after surgery showed that there was no significant difference in the survival time of patients with high or low POLD3 expression(P>0.05).Conclusion The expression level of POLD3 is decreased in HCC patients,and it may play an important role in the pathogenesis and progression of HCC,thereby providing new ideas and targets for the diagnosis,treatment and prognosis of HCC.
