基于气机升降理论探讨《医学衷中参西录》中反流性食管炎相类病症的中医辨治

摘　　要：从脏腑失职,气机逆乱及调畅气机,重在降逆两方面入手探讨《医学衷中参西录》中与反流性食管炎相类病症的中医辨治方法,指出临床目前已证实反流性食管炎与脏腑气机升降失常密切相关。《医学衷中参西录》中反流性食管炎相类病症如嗳气、嘈杂、呕逆、反胃、饮食有碍等症状主要与脾胃、肝、心肺、肾的气机升降失常有关,病因病机为脾胃虚弱,枢机不利;肝胃不和,升降失常;心肺阳虚,宣通不及;肾虚不摄,气化失司;治疗方面张锡纯通过药物调整脏腑气机升降,使脏腑恢复其生理特性,主要有补中降逆,调肝降逆,温通降逆,补肾降逆等。The methods of TCM syndrome differetiation for reflux esophagitis(RE)similar disease and symptom in Yixue Zhongzhong Canxilu are discussed from two aspects:visceral dysfunction,inverse Qi movement,and regulating Qi movement,reducing the inversion,it has been confirmed that RE was closely related to abnormal ascending and descending of visceral Qi movement in clinic.RE similar disease amd symptom in Yixue Zhongzhong Canxilu such as belching,gastric noise,regurgitation,dietary obstruction were mainly related to abnormal ascending and descending of Qi movement of spleen and stomach,liver,heart and lung,and kidney,the cause and the pathogenesis were weakness of spleen and stomach causing cardinal inhibition;iver and stomach disharmony,abnormal ascending and descending;Yang deficiency of heart and lung,failing to diffuse and dredging;kidney deficiency failing to absorbing,uncontrolled Qi transformation.Zhang Xichun,the writer,adjust ascending and descending of visceral Qi movement with drugs to restore the physiological functions of the viscera in the treatment,and the methods are tonifying the middle and descending the inverse,regulating liver and descending the inverse,warming and dredging to reduce the inverse,tonifying the kidney and descending the inverse.
