疏肝降脂汤治疗肝郁脾虚型非酒精性脂肪肝44例

摘　　要：目的:观察疏肝降脂汤治疗肝郁脾虚型非酒精性脂肪性肝炎(nonalcoholic steatohepatitis,NASH)的临床疗效。方法:将88例患者按随机数字表法分为观察组和对照组,每组44例。两组均予以常规干预治疗。对照组口服甘草酸二铵肠溶胶囊,每次3粒,每日3次。观察组在对照组基础上口服疏肝降脂汤,每日1剂,早晚分服,两组均连续治疗3个月。比较两组肝功能、血脂水平及临床疗效,血清肿瘤坏死因子α(tumor necrosis factorα,TNF-α)、白细胞介素6(interleukin-6,IL-6)水平。结果:总有效率观察组为95.45%(42/44),对照组为77.27%(34/44),两组比较差异有统计学意义(P<0.05)。丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨基转移酶(aspartate aminotransferase,AST)、γ-谷氨酰转肽酶(gamma-glutamyl transferase,γ-GT)、总胆红素(total bilirubin,TBIL),甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白(low density lipoprotein,LDL)以及血清TNF-α、IL-6水平,治疗前后两组组内比较,差异有统计学意义(P<0.05);治疗后两组组间比较差异也有统计学意义(P<0.05)。结论:在常规西医治疗的基础上,以疏肝降脂汤治疗肝郁脾虚型NASH疗效确切,其机制可能与下调患者体内TNF-α和IL-6有关。Objective:To observe clinical effects of liver-soothing lipid-lowering decoction in the treatment of nonalcoholic steatohepatitis(NASH)of liver depresion and spleen deficiency pattern.Methods:All 88 patients were randomized into the observation group and the control group by numerical form,44 cases each groups.Both groups were treated by conventional intervention.The control group took diammonium glycyrrhizinate enteric-coated capsules orally,three pills every time,three times each day.The observation group took liver-soothing lipid-lowering decoction based on Western Chinese that the control group received,one dose each day,taking in the morning and evening,both groups were treated for three months consecutively.To compare liver function,the levels of blood lipid and clinical effects between both groups,detect the levels of TNF-αand IL-6 in the serum.Results:Total effective rate of the observation group was 95.45%(42/44),higher than 77.27%(34/44)of the control group,and the difference had statistical meaning(P<0.05).The difference had statistical meaning in the levels of liver function indexes including ALT,AST,γ-GT and TBIL,blood lipid indexes including TG,TC,LDL and others,the levels of TNF-α,IL-6 and others within two groups before and after treating(P<0.05);the difference showed statistical meaning between both groups after treating(P<0.05).Conclusion:On the foundation of conventional Western medicine,liver-soothing lipid-lowering decoction could obtain definite effects in the treatment of NASH of liver depresion and spleen deficiency pattern,its down-regulation of TNF-αand IL-6 might be related to its therapeutic effects.
