儿童及青少年运动性晕厥的病因及临床特征分析

摘　　要：目的分析儿童及青少年运动性晕厥的病因及临床特征。方法选择2014年4月至2019年4月在柳州市妇幼保健院儿科收治的运动性晕厥患儿53例,对确诊患儿进行病因分类,分析导致晕厥与运动的关系、临床表现、辅助检查,对心源性及神经介导性晕厥患儿资料进行统计学处理,试图发现两者的特征性表现。结果 53例运动性晕厥患儿中,心源性晕厥13例(24.5%),自主神经介导性晕厥31例(58.5%),不明原因晕厥9例(17.0%)。心源性晕厥多见于剧烈运动中,有家族史,伴有抽搐或大小便失禁,心电图可见异常结果。自主神经介导性晕厥多见于运动结束后发生。结论自主神经介导性晕厥是儿童及青少年运动性晕厥最常见原因之一,其次为心源性晕厥。对于运动性晕厥患者,晕厥发生于运动中还是运动后、是否有伴随症状对病因诊断意义重大。Objective To analyze the etiology and clinical characteristics of exercise⁃related syncope in children and adolescents.Methods 53 children/adolescents diagnosed exercise⁃related syncope were retrospectively reviewed in Liuzhou Maternal and Child Healthcare Hospital from April 2014 to April 2019.All the patients were classified by etiologically.To analyze the rellation among the sports project,syncope during or after exercise,clinical manifestation and assistant examinations.Results Among these,13 cases(24.5%)were cardiogenic syncope(CS),31 cases(58.5%)were neurally mediated syncope(NMS)and 9 cases(17.0%)were unexplained syncope.Syncope was more occured in strenuous family history,convulsions or fecal incontinenceor abnormal ECG.Auto⁃nomic nerve⁃mediated syncope usually occurs after exercise.Conclusions The VVS is one of the most common cause of exercise⁃related syncope in children and adolescents,followed by cardiogenic syncope.For exercise⁃relat⁃ed syncope,whether the syncope occurs during or after exercise and whether there are concomitant symptoms are of great significance to the etiological diagnosis.
