摘 要:目的探讨红细胞体积(MCV)与红细胞含量(MCH)联合检测在地中海贫血与缺铁性贫血鉴别诊断中的应用价值。方法回顾性分析我院2017年6月~2019年8月收治的74例贫血患者的临床资料,将确诊为地中海贫血的37例患者设为A组,确诊为缺铁性贫血的37例患者设为B组,另选取同期我院37例健康体检者设为C组。测定并比较三组MCV及MCH水平,以血涂片形态检查结果为金标准,比较单项检测与联合检测鉴别诊断地中海贫血与缺铁性贫血的价值。结果三组MCV与MCH水平比较,差异有统计学意义(P<0.05);C组MCV与MCH水平高于A组、B组,差异有统计学意义(P<0.05),B组MCV与MCH水平低于A组,差异有统计学意义(P<0.05);MCV鉴别诊断两病的准确度为91.89%,MCH检测准确度为93.24%,联合检测诊断两病的准确度为97.30%。联合检测的准确度略高于MCV及MCH单项检测,但差异无统计学意义(P<0.05)。结论MCV与MCH联合检验的诊断准确度较单项检验略高,在鉴别诊断地中海贫血与缺铁性贫血中推荐使用联合检测的方式。Objective To explore the application value of the combined detection of red blood cell volume(MCV)and red blood cell content(MCH)in the differential diagnosis of thalassemia and iron deficiency anemia.Methods The clinical data of 74 anemia patients admitted to our hospital from June 2017 to August 2019 were retrospectively analyzed.37 patients diagnosed with thalassemia were assigned to group A,and 37 patients diagnosed with iron deficiency anemia For group B,another 37 healthy people in our hospital during the same period were selected as group C.The MCV and MCH levels of the three groups were measured and compared.The results of blood smear morphology examination were used as the gold standard to compare the value of single test and combined test in the differential diagnosis of thalassemia and iron deficiency anemia.Results Compared with the levels of MCV and MCH in the three groups,the difference was statistically significant(P<0.05);The levels of MCV and MCH in group C were higher than those in groups A and B,the difference was statistically significant(P<0.05).The MCV and MCH levels in group B were lower than those in group A,the difference was statistically significant(P<0.05);the accuracy of MCV differential diagnosis of the two diseases was 91.89%,and the accuracy of MCH detection was 93.24%,the accuracy of the joint detection and diagnosis of the two diseases is 97.30%.The accuracy of the combined test is slightly higher than that of the MCV and MCH single tests,but the difference is not statistically significant(P<0.05).Conclusion The diagnostic accuracy of the combined test of MCV and MCH is slightly higher than that of a single test.It is recommended to use the combined test in the differential diagnosis of thalassemia and iron deficiency anemia.
