摘　　要：目的检测胃癌组织中EB病毒(EBV)表达和磷脂酰肌醇3激酶α(PIK3CA)基因突变情况,探讨EB病毒相关性胃癌的临床分子病理特征。方法选取山西省肿瘤医院2017—2019年胃癌手术标本356例,采用EBER原位杂交法检测EBV,荧光聚合酶链反应(PCR)法检测PIK3CA基因突变,统计分析EBV感染与胃癌临床病理特征及PIK3CA基因突变之间的关系。结果356例胃癌组织中检出EBV阳性16例,阳性率4.5%;结合临床病理特征分析,EBV感染可能与患者年龄有关(χ^2=4.777,P<0.05),而与性别、Lauren组织学分型、淋巴结转移、脉管浸润、神经累犯无关;基因检测发现在EBV阳性组中有5例发生PIK3CA基因突变(31%),而EBV阴性组中只检测到1例突变(3%),2组差异有统计学意义(P<0.05);相关性分析结果表明胃癌组织中PIK3CA基因突变与EBV感染呈正相关(r=0.401,P<0.05)。结论中国人群中EBV相关性胃癌的发生率较低,且与PIK3CA基因突变相关,针对PIK3CA的靶向治疗药物可能对此类型胃癌具有潜在的临床应用价值。Objective To examine the expression of Epstein-Barr virus(EBV)and mutation of phosphatidylinositol 3-kinase catalytic alpha(PIK3CA)gene in gastric cancer tissues,and investigate the clinical and molecular pathology characteristics in EBV-associated gastric cancer.Methods A total of 356 surgically resected specimens of gastric cancer tissues in Shanxi Cancer Hospital between 2017 and 2019 were enrolled in this study.EBV was examined by in situ hybridization,and mutations in PIK3CA gene were evaluated by fluorescence PCR.Statistical analysis was used to identify the relationship between EBV infection,clinicopathological characteristics and PIK3CA mutation in gastric cancer.Results EBV infection were detected in 16 out of 356 gastric cancer tissues(4.5%).Analysis according to the clinicopathological characteristics showed that EBV infection was significantly associated with patient age(P<0.05),but not with gender,Lauren histological classification,lymphatic metastasis and vascular invasion.The mutation of PIK3CA gene were detected in 5 out of EBV positive specimens(31%),and only 1 outof EBV negative specimens(3%),with significant difference(P<0.05).Correlation analysis showed that PIK3CA gene mutation was positively correlated with EBV infection in gastric cancer tissues(r=0.401,P<0.05).Conclusion The incidence of EBV associated gastric cancer is low in Chinese population,and may be related to PIK3CA gene mutation.PIK3CA targeted drugs may be clinically potential for this type of gastric cancer.
