房角分离联合白内障超声乳化及人工晶体植入术治疗慢性闭角型青光眼疗效分析

摘　　要：目的探讨房角分离联合白内障超声乳化及人工晶体植入术治疗慢性闭角型青光眼(CACG)疗效及并发症。方法我院诊治的98例CACG患者,根据手术方式分为对照组48例和研究组50例,对照组采用白内障超声乳化及人工晶体植入术,研究组采用白内障超声乳化及人工晶体植入联合房角分离术。比较两组手术前后视力、眼内压、中央前房深度(ACD)、视野变化及术中和术后并发症情况。结果两组术后视力高于术前,眼压低于术前,且研究组与对照组差异明显(P<0.05);两组术后ACD值高于术前,且研究组高于对照组(P<0.05);与对照组相比,研究组术后4、8个月视野平均光敏度(MS)较术前增高明显,视野指数(MD)绝对值较术前降低明显(P<0.05);两组术后并发症发生率比较差异无统计学意义(P>0.05)。结论房角分离联合白内障超声乳化及人工晶体植入术治疗CACG可有效提高患者视力,降低术后眼压,改善视神经以及视野功能,术后并发症少。Objective To investigate the curative effect and complications of goniosynechialysis combined with cataract phacoemulsification and intraocular lens implantation(ICL)in the treatment of chronic angle-closure glaucoma(CACG).Methods Ninety-eight CACG patients who were diagnosed and treated in our hospital from June 2016 to June 2019 were divided into control group(n=48)and study group(n=50)according to different surgical methods.The control group was treated with cataract phacoemulsification and ICL while the study group was additionally treated with goniosynechialysis based on the control group.Changes in visual acuity,intraocular pressure,anterior chamber depth(ACD)and visual field before and after surgery were compared between the two groups.The intraoperative and postoperative complications were statistically analyzed.Results After surgery,visual acuity was significantly increased while intraocular pressure was significantly decreased in both group and the differences between the two groups were statistically significant(P<0.05).After surgery,ACD in both groups was significantly increased,and the increase in the study group was significantly higher than that in the control group(P<0.05).Compared with the control group,mean sensitivity(MS)of visual field after 4 and 8 months of surgery was significantly higher than that before surgery while absolute value of mean defect(MD)was significantly lower in the study group(P<0.05).There was no significant difference in incidence of postoperative complications between the two groups(P>0.05).Conclusion Goniosynechialysis combined with cataract phacoemulsification and ICL for CACG can effectively improve the visual acuity of patients,reduce the postoperative intraocular pressure,improve the optic nerve and visual field function,and postoperative complications are few.
