基于脑-肠轴“从肠治脑”论治急性脑中风痰热腑实证意识障碍

摘　　要：痰热腑实证是急性脑中风的重要病理阶段,能引起患者不同程度的意识障碍。作为治疗痰热腑实证最重要的法则--化痰通腑法,其治疗急性脑中风痰热腑实证所致意识障碍的机制未完全清楚。本研究结合脑-肠轴研究成果,以脑-肠轴学说为核心,从脑肠互动探讨脑肠肽、肠道菌群在痰热腑实证意识障碍中的重要作用。根据"从肠治脑",运用化痰通腑法通过泻"浊",调节脑肠肽、肠道菌群,借以平衡脑肠轴的功能,从而阐明其治疗急性脑中风痰热腑实证"浊邪害清"所致意识障碍的机理。同时也从脑肠肽、肠道菌群多靶点为突破口,为脑-肠相关疾病的防治提供理论基础与临床依据。Phlegm heat excess syndrome is an important pathological stage of acute cerebral apoplexy,which can cause disturbance of consciousness to different degrees. The mechanism of Huatan Tongfu method,as the most important principle in its treatment,is not completely clear. Based on the current research results of brain-gut axis and the theory of brain-gut axis as the core,this paper discusses the important role of brain gut peptide and gut flora in the disturbance of consciousness with phlegm heat excess syndrome from the perspective of brain gut interaction. According to the principle of"treating brain from intestinal",the method of Huatan Tongfu is used to regulate brain gut peptide and intestinal flora by removing"Zhuo",so as to balance the function of brain-gut axis and clarify its treatment mechanism on consciousness disorder caused by acute cerebral apoplexy with phlegm heat excess syndrome. It also provides theoretical and clinical significance for the prevention and treatment of brain-gut related diseases from the perspective of brain-gut peptide and multiple-target intestinal flora as the breakthrough point.
